- Identify the category (or categories) of disease prevention or health promotion change processes you are addressing.
- Describe the level (or levels) of the socioecological model you are addressing with this intervention.
- Describe the intervention. (e.g., communications campaign to create knowledge and motivate action, provision of a service to reduce disease, an effort to reduce barriers to care, etc.).
- Describe what theory or theories you are using in the design of your intervention and why you think they are appropriate for the population with whom, and setting in which, you are working.
- Create a logic model or flow chart to show how theory applies to your intervention, including an illustration of the social level (or levels) at which this intervention will occur.
- Discuss how you propose to involve the community in the assessment of need and design of the intervention.
- Quantitative and qualitative data analysis—Include key data measures that highlight the importance of participating in a research study.
