Hypertension

Hypertension is one of the most common cardiovascular conditions in the United

States and worldwide. So severe is this disease that it may result in premature death, stroke,

chronic kidney failure, coronary artery disease, and myocardial infarction if untreated.

Statistics indicate that 25% of adults in western nations are hypertensive (Oparil et al., 2018).

However, most of them are unaware of having this problem. Hypertension refers to elevated

blood pressure levels. Current criteria for diagnosing hypertension include having systolic

blood pressure readings of more than 130mmHg and diastolic blood pressures of higher than

80mmHg at more than two reproducible measurements. Additionally, systolic blood

pressures of between 130-139mmHg and diastolic blood pressures of 80-89mmHg are

classified as prehypertension (Hinkle & Cheever, 2018).

The risk of hypertension and other cardiovascular diseases increases with aging,

although other factors significantly predispose one to these diseases. Individuals from the

African American ethnicity are highly predisposed to having hypertension in their lives

compared to other ethnic groups. Furthermore, being overweight or obese, poor dietary habits

like taking too salty foods, cigarette smoking, excessive alcohol consumption, and being

stressed increase the chances of one having high blood pressure. Moreover, hypertension is

regarded as a silent killer as it often lacks any signs and symptoms except elevated blood

pressure readings, although symptoms may appear in case of vascular damages (Hinkle &

Cheever, 2018). Some of the symptoms of severe hypertension include hemorrhages,

arteriolar narrowing, and papilledema. Therefore, prevention through lifestyle modification

and timely diagnosis and treatment are crucial for managing hypertension and avoiding

complications.

Patient Description

Patient DS was admitted on 3/2/2022 with complaints of elevated blood pressures,

headache, nausea, and noisy abdominal area. The patient was oriented x4. The code status

was full code with a temperature of 36.6, BP of 150/94, SpO2 of 100%, and heart rate of 79.

The weight of the patient was 190 lbs and height of 5’7. The patient took hypertensive

medications at home, and he was given hydralazine (IV) at the hospital.

Analysis

While the care provided to DS is consistent with the guidelines, additional

interventions are required to achieve optimal blood pressure control. First, the nursing staff

needs to conduct a comprehensive physical examination of the patient to identify any signs of

organ damage by palpating peripheral pulses for weak, delayed, or absent pulses indicative of

severe peripheral disease (Hinkle & Cheever, 2018). Laboratory tests like urinalysis, kidney

function tests, and blood chemistry are also significant for determining organ damage

secondary to hypertension.

Second, evidence-based guidelines recommend the utilization of both non-

pharmacological and pharmacological therapies for the management of hypertension. Clients

like DS having the risk factors of hypertension such as smoking, being overweight, being

male, and having an advanced age should be advised by the nurse on lifestyle modifications.

The appropriate lifestyle changes for the at-risk patients include engaging in physical

activities, weight loss, smoking cessation, and modifying dietary choices (Oparil et al., 2018).

Specifically, the Dietary Approach to Stop Hypertension (DASH) diet helps decrease

elevated blood pressure when coupled with weight loss in susceptible clients. The DASH

eating plan involves a high intake of vegetables, fruits, and low-fat products. Restricting

sodium intake and raising potassium intake from the diet are also instrumental for high blood

pressure management (Hinkle & Cheever, 2018). Thus, DS should be encouraged to avoid a

sedentary lifestyle for optimal blood pressure control.

Pharmacological options exist for the management of hypertension. The first-line

drugs for controlling hypertension and lowering the risk of cardiovascular conditions include

angiotensin-converting enzyme (ACE) inhibitors, angiotensin receptor blockers (ARBs),

calcium channel blockers (CCB), and thiazides. Therapy entails combining two or more of

these drugs with a gradual increase of doses to achieve blood pressure of less than 130/80

mmHg. ACE inhibitors lower blood pressure by hindering the conversion of angiotensin I to

angiotensin II consequently decreasing the cumulative peripheral resistance. Common ACE

inhibitors include enalapril, ramipril, captopril, and lisinopril. However, nurses should be

cautious of angioedema, a potentially life-threatening complication of ACE inhibitors

therapy. Similarly, ARBs lower peripheral vascular resistance by inhibiting the function of

angiotensin II at the receptor. Some common ARBs include losartan, candesartan, valsartan

and Olmesartan (Hinkle & Cheever, 2018). During ACE inhibitors and ARBs therapy, nurses

need to monitor for hyperkalemia which is a potential complication from these medications.

Summary

Hypertension is a life-threatening illness that requires prompt management to reduce

complications and prolong life. The risk of being hypertensive usually increases with age.

Also, it is common among African Americans, males, smokers, alcoholics, obese, overweight

individuals, and people who do not exercise adequately. Thus, lifestyle modifications by

losing weight, adopting the DASH diet, engaging in physical activities, quitting cigarette

smoking, lowering alcohol intake, and avoiding stress help manage hypertension (Oparil et

al., 2018). Similarly, proper compliance to antihypertensive medications such as ACE

inhibitors, CCB, ARBs, and beta-blockers is instrumental for lowering elevated blood

pressures.

Reflection

This assignment was an eye-opener for me regarding hypertension and its

management. Specifically, I learned about the changed system for diagnosing hypertension. I

learned that current guidelines for diagnosing hypertension include staging hypertension from

normal to stage 2. Mainly, systolic blood pressures of 120-129mmHg and diastolic blood

pressures of greater than 80mmHg are classified as elevated BP, unlike the previous

classification of prehypertension. This revision in classification aims to portray the increasing

risk of cardiovascular illnesses with increasing systolic or diastolic blood pressures. This new

information made me feel great about choosing the nursing profession as I can aid in the

prevention of hypertension through proper health education to clients with elevated blood

pressures. I intend to apply the information learned from this assignment in my future nursing

career. I will also encourage patients to frequently check their blood pressure and modify

their lifestyles to reduce the risk of hypertension and other cardiovascular conditions. I

realized that both pharmacological and lifestyle changes are essential in managing

hypertension.
