MEDICAL PRE EMPLOYMENT QUESTIONAIRE SCENARIO. You are the H&S manager for Choco Blocko. You are an American Company who are looking to staff their new manufacturing plant in Lesserpool. Lesserpool is a productive area, attracting large amounts of European funding. Many new firms have opened up production centres around the area. Because of the potential implications from Brexit previous advertisements in attracting potential employees was unsuccessful. Access to the site is easily obtained because road transport links were planned as part of the area regeneration. You have held preliminary interviews and have found that Mr A Jobby is available for employment and there are several vacancies he may be suitable for. Mr. A Jobby does have a driving licence but does not have a forklift truck licence. Human Resources have sent you the Job Recommendation form to respond to. The jobs are: Production Line Operative Monitoring of chocolate machine line. This work will involve an eight hour shift stood by the side of a machine carrying packed and wrapped boxes of chocolates to a containment area. The work is mainly process driven with little supervision. The main problem is that the conveyer machine can jam causing backlog. This may happen at different frequencies in time dependant upon the type of chocolate box being packed. Factory mail and general delivery. The role for this work is to deliver mail and all other parcels to various areas of the company. The person operating the role must be of average fitness. The general movement of mail and other post room duties may involve movement of heavy – (no more that 35kg packages over long distances). Chocolate mixing operative. Chocolate beans are automatically loaded in a grinding machine then moved along an automatic system where it is the automatically added to other ingredients for making the base liquid chocolate mix. This post involves unblocking the bean loading machine. This involves physically turning off the machine, and using a prod to unblock the jammed beans. General attendant. This role involves the general cleaning and tidying up of the internal factory area. This will involve working with approved cleaning materials. There are several specific areas where cleaning will be required. The role will be for work in a specific area, The chocolate grinding and mixing room, the store and packaging are, the box and paper assembly areas. There is also a post for a general assistant /cleaner in the office block. Duties will include security, general cleaning of offices and associated toilets. This post also involves the escorting of guests to conference rooms or to the company individuals. Store Operative Store operatives will be involved in all general store activity. Placing of orders, loading of vehicles, placing identification labels on orders. The operatives will be expected to be able to
drive and operate a forklift truck (FLT). The operatives will be responsible for basic maintenance of FLT’s, including daily pre user checks, refuelling or recharging of vehicles, replacement of lights etc. Your task is to make recommendations to the HR Department about the role that would suit Mr Ajobby and what reasonable adjustments could or may need to be made to ensure no detriment is caused to his future health. HR have told you that they only just had enough applicants to fill 90% of the vacancies advertised. The factory must be fully operation within 3 weeks. You are to respond to the request from HR, detailing only the job you are recommending him for. You must also in your response to the request from HR, inform HR of any additional control measures you advise that does need to be taken that will enable Mr A Jobby to undertake the role you are recommending him for.
