Requirements

• Every section of Chemical RA Form to be completed. If ‘Not Applicable’ – mark as N/A

• Mark relevant pictograms

• Include correct Hazard Phrases, Precautionary Phrases and Risk Phrases per SDS

• Identify whether license/permit required from legal authorities based on chemical and

quantity stored

• Be specific on PPEs required eg. nitrile gloves to use

• Identify any specific incompatibilities when storing the chemical at site (based on other

chemicals stored)

• State requirements for IH surveys and medical surveillance per WSH Act and regulations

• State existing control measure in place when storing, handling and disposing chemical

• Provide additional control measures to be implemented (provide at least 3 measures.

Use upstream risk controls as far as possible)
