SRM Assessment 1 – Workplace Equipment Risk Assessment

Maximum marks: 20 marks

Requirements

• Scope of the RA is clearly defined per scenario given

• Hazard identification

– Application of 4M and 1 E

– Identification of foreseeable safety and health hazards of the activity

– Description of nature of the hazard and injury/illness is clear and specific on how the

hazard arose

– Identification of foreseeable persons at risk

• Risk evaluation

– Include criteria for assessing the severity, likelihood and risk matrix

– Usage of reasonable severity and likelihood rating based on foreseeable hazards

• Risk Control

– Identification of existing controls. ( at least 2 per hazard)

– Identification of additional controls (at least 2 per hazard). More controls if risk is

medium or high risk

– Application of hierarchy of control. More marks will be given for upstream risk control

measures for additional controls (eg. elimination, substitution and engineering controls)

– Identification of specific action person (with designation) and implementation date

• Completion of RA

– RA Form completed, showing clearly person conducting assessment and dates

– Check for similiarity (<30%).
