Group Proposal

RJ’s foundation is a foundation for drug addict from age 15-70 that offers outpatient treatment for individuals with alcohol and substance addiction. This program includes family support, individual therapy, individualized treatment and more.

Unmet Needs

According to Hellen Keller “Character cannot be developed in ease and quiet. Only through experience of trial and suffering can the soul be strengthened, ambition inspired, and success achieved.” The mission of RJ’s foundation is to help other stay away from drugs, alcohol, and all substance abuses. Also, to provide quality substance abuse prevention, intervention, and treatment services leading to improved health, wellness.

Purpose

· To help client achieve their goals.

· Advocate, Intervene, and prevent.

· Prevent the use and misuse of drugs and the development of substance use disorders in the community.

· Build another community, with another generation.

Structure

· Group size: 5 members per group

· Number of meetings: 24 meeting for next 12 months

· Frequency of the meeting: Each group will meet twice a month.

· Location of the meeting: The meeting will take place via zoom and in person at the conference room 1001 in the library. Group member will be seated by age.

· Schedule of the meetings: Every Friday at 2pm -5pm.

· Length of meeting: two hours per meeting and extra one hour for activities.

Composition

· Age/Sex of members: Each participant must enroll in the program and must be 15-70 yrs. old. In RJ’s foundation we do not discriminate all gender and sex can enroll.

· Social/emotional: The majority of the members of the group have been sexually abused by their partner, and the goal of the group circle meeting is to embrace them, help them forget their past, and emotionally support them. In the meeting, we will present some cases and invite everyone to participate in solving them as a way to assist them in the future when they face similar problems.

· Behavior patterns relevant to members: The leader should be approachable and affirming rather than distant or critical because many clients with histories of substance addiction grew up in households with little protection, safety, or support. Especially in the first phases of treatment, the group leader needs to understand that the participants are weak and in need of assistance. The examination of additional crucial characteristics for a group leader follows that. Above all, the group's leader must recognize that he is in charge of making a number of choices as the team advances.

Orientation

To communicate with members, we will use the foundation's website and other social media platforms. All new members will register online at the website or in person at the lobby, where staff will be happy to assist them in signing up for the program. In the event of an event, each member is required to bring at least one friend. We value our members at this foundation, and we do not share information with anyone outside of the program because we believe in confidentiality and respect.

Foundation Policy

The foundation policy must be followed by all members, and each member must add two policies. The foundation Policy unit is in charge of the following Medicaid policy areas: Beneficiary eligibility, Supplier enrollment, Benefit submission and repayment, including Health coverage crossover and other third-party payment policy, Interagency agreements, and other third-party associated with increased rates.

The Policy Foundation is also in charge of the following specialized programs.

Pre-admission screening and resident review process, as well as oversight of the Medicaid responsibilities of the children's multidisciplinary assessment team. Privacy and security are a valuable and important and fundamental understanding between staff and members. Members' personal privacy and equipment will be protected and supported unless a constitutional or ethical standard requires otherwise. For instance, when a member demand and gives consent to the release of information; or when there is a reasonable suspicion of child, elder, or dependent adult abuse (including the upload and download, payment, and/or observing. There is a distinct and immediate danger from the client to someone else.

Coordinators

The CEO will lay the groundwork for the organization's operational plans and procedures. The above potential impacts of all foundation components and departments. The CEO will also focus on ensuring that key both internal and external variables are constantly communicated to the organization's leadership. The manager will be in charge of reporting everything to the CEO. Supervisors are in charge of recruiting new employees and program participants. They will contact the manager for final approval after they have approved everything. The coordinators are in charge of recruiting group members, organizing events for them, and assigning each member to a therapist for counseling. The therapist will evaluate and manage mental illnesses. Individualized treatment plans are developed based on the needs and circumstances of the members. Members are seen twice a month for counseling and treatment, and treatment plans are adjusted as needed. Conducts ongoing evaluations of members' progress.

Evaluation

· Group attendance will be taken in the check in.

· Therapist will provide feedback on every single member for improvement.

· A test will take place every two months for members improvement.

· At the end of the 12months session each member will give a brief summary on how the program help them.
