Mission

Our mission is to improve the health and wellbeing of our patients by providing our

patients with the best and most comprehensive care. We aim to provide a family-centered

environment in which our patients can feel safe and comfortable. We pride ourselves on the

relationships that we build with our patients and their families. As a practice, we are committed

to continuing education so that we can provide care that is centered in evidence-based practice.

We believe in disease prevention and aim to help our patients foster a healthy lifestyle. We aim to

be efficient and cost-efficient while meeting the needs of our patients. Our physicians and nurses

are recognized for their compassion, experience, knowledge, and commitment to our patients.

Vision

Our vision is to be the healthcare provider of choice for the community. We will

transform the healthcare experience of our patients by providing care that considers their medical

diagnosis, cultural, and spiritual beliefs. We believe that each patient is an individual and should

be regarded as such and we will go above and beyond to meet the needs of each patient that

walks through the doors of our practice.

 Collaboration: Physicians, nurse practitioners, and healthcare staff are always in

communication amongst each other in order to diagnose and treat patients

effectively.

 Quality: Our health care team provides high quality care by assessing and

treating patients with meticulous detail and educating them on their health based

on their level of understanding.

Goal

Our goal is to provide affordable care to a diverse population of patients to ensure their

needs are met and that they are educated on the importance of their health. Some patients have a

language barrier and it is vital to ensure that they have a proper level of understanding regarding

their health. We educate our patients, provide quality health care, and ensure that they are

following up on their visits.

Practice

We are a small primary care office named “Medical Associates Plaza PC” located on

Church Avenue in Brooklyn, New York. Our practice services people in the community aged 18

years and older. The patients seen in our practice are diverse, primarily immigrants from the

Caribbean, migrating from countries such as Haiti, Jamaica, Grenada, and St. Vincent. We offer

preventive care and provide services such as: physical examinations, immunizations, pain

management and management of chronic illnesses. On average, our practice sees 150 to 200

patients daily. Our practice consists of six exam rooms, one for each provider. The physicians see

an average of four patients an hour and the nurse practitioners see three patients an hour. We are

open Monday through Friday from 9:00 AM to 5:00 PM and closed on all eleven major holidays.

Our practice hires eleven full-time employees: two physicians, four nurse practitioners,

two medical assistants, two secretaries, and one medical biller. All primary providers are

expected to be in the clinic on all working days as well as the two full-time medical assistants

and the two secretaries. We employ one part-time medical assistant working 20 hours a week and

one housekeeper working 4 hours per day, after hours.

Expenses

Based on our research, salaries are obtained from Glassdoor (2021) utilizing location and

employment type. Employees' expenses are calculated based on 25% of the annual salary of all

full-time employees. Using a medical office square footage calculator, we estimate needing

approximately 2000 sq. ft which would consist of six examination rooms, one office, a break

room, reception, and a waiting area (O’Donnell Commercial Real Estate, Inc., 2021). Prices of

office and medical supplies are obtained from major suppliers and multiple online sources such

as Staples, Best Buy, Medical Device Depot, and Vitality Medical among others. Quest

Diagnostics will provide laboratory supplies as they are our chosen provider to run the clinic’s

lab works.

The biggest expenses of the clinic are the vaccinations. Centers for Disease Control and

Prevention [CDC] (2021) recommended that adults get influenza vaccines annually. Other

vaccines that are recommended to adults and geriatric patients include Td/Tdap, MMR, Varicella,

Zoster, HPV, Pneumococcal, and Hepatitis A&B (CDC, 2021). The tables below show the

breakdown of our primary care office’s expenses and revenue.
