State Required CORE Assessment for Teamwork

· For this assessment, a team consists of at least two members – these are people that you have worked with in this class in an academic activity, such as Discussion Board, Peer Review, email correspondence, or other interaction.

· You should have engaged in at least two team interactions

· Reference one (or more) of the following three sources of evaluation, using the Teamwork Value Rubric as a guideline

· Student’s self-reflection about his or her contribution to the team’s functioning

· Evaluation or feedback from fellow team member about student’s contribution to the team’s functioning

· The evaluation of an outside observer regarding student’s contribution to a team’s functioning (the instructor, tutor, workshop presenter)

Questions for the student

3. Think about your role with your team member(s). What role did you play? What were you best at contributing?

4. On a scale of 1-4 with 1 being “poor” and 4 being “excellent,” how would you rate your contribution to the team/study group?
