Discussion: Diversity and Health Assessments
Running head: BUILDING A HEALTH HISTORY 1
BUILDING A HEALTH HISTORY 4
I want to note that the assignment is meant to assign a patient profile for you to work on for this week. You will find the new patient profiles listed below. Take note, you will need your assigned new patient profile to complete the Discussion for this week.
As students, consider the impact of functional assessments, diversity, and sensitivity in conducting health assessments. Please also explore various assessment tools and diagnostic tests that are used to gather information about patients’ conditions and examine the validity and reliability of these tests and tools. You will examine assessment techniques, health risks and concerns, and recommendations for care related to patient growth, weight, and nutrition.
Consider how you would build a health history for the patient. What questions would you ask, and how would you frame them to be sensitive to the patient’s background, lifestyle, and culture? Develop five targeted questions you would ask the patient to build his or her health history and to assess his or her health risks.
As per Walden University Policy, make sure you are avoiding any potential academic integrity issues.
If your Last Name is from A through M – Please Take Case Study 3.
If your Last Name is from N through Z – Please Take Case Study 4.

He tells you he is afraid that he will not get into heaven if he continues in this lifestyle. He is not taking any prescriptions medications and denies drug use. He has a positive family history of diabetes, hypertension, and alcoholism.
