Transtheoretical Model (Ex.1)—1 point
Health Contract (Ex. 2)—2 points
· 3 Scholarly Journal Articles & 3 summaries
8 points each (24 points total)
Scholarly journal article must be shown in paper copy to the instructor by 5/14/19
For Full Credit:
· Summaries (Total of 3 from 3 articles) must be 1.5 page each, double-spaced, TYPED.
· Each summary must be based on a current scholarly journal article
That is pertinent to your behavior change.
· The journal article must be at least 4 pages in length and
· A hard paper copy of the article must be shown to the instructor on 5/14/19 .
· A work cited page and in-text citations of the article must be included.
· Plagiarized summaries will receive no credit.
If you plagiarize even one summary the entire project will receive a ZERO.
It is in your syllabus.
· Your Personal Journal Entries— (12 points total)
For Full Credit: A minimum of 12 journal entries are required they must be double-spaced. Include specific activities, behaviors, attitudes and thoughts about the project.
· Also you must state the progression of the Transtheoretical model and the stages.
· Unit 10, Exercise 1—(10 points total)
· For Full credit: you must type up the answers to the questions, double-spaced.
· You must respond to questions thoroughly.
*** EVERYTHING WILL BE SUBMITTED HARDCOPY INCLUDING THE ARTICLES THAT YOU PRINTED IN THE RIGHT ORDERS ACCORDING TO THIS CHECKLIST AND ALSO THE PROJECT EXAMPLE THAT I SHOWED YOU GUYS. THERE IS NO ONLINE SUBMISSION ON BLACKBOARD SO DON'T ASK ME.
