You will choose ONE video case study for your written assignment. Please use the Comprehensive Psychiatric Evaluation Template and Comprehensive Psychiatric Evaluation Exemplar as guides for your written assignments. For full credit, please read and follow the RUBRIC. If any information is not present, then you need to discuss why that information needs to be included. For example, if the video nor case history report has no mention of the patient’s allergy then please write that the allergy was not mentioned and why it is significant to include in your evaluation.

The Comprehensive Psychiatric Evaluation Exemplar explains in detail the components of the Comprehensive Psychiatric Evaluation Template. Both the template and exemplar are located under the “Required Readings”

For example, the Exemplar explains how to formulate a Chief Complaint (CC). Typically, CC is verbatim from patient ie “I am anxious.”

In addition to reviewing the template and exemplar, please read the rubric carefully in order not to lose points.

Reflect on this case. Discuss what you learned and what you might do differently. Also include in your reflection a discussion related to legal/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), health promotion and disease prevention taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).

Please do not just indicate "not applicable." If the physical exam is not conducted then please indicate what physical exam you will perform. Psychotropic medications can affect various systems i.e. neurological. For example, you can examine serotonin syndrome if you are prescribing SSRIs. One of the cardinal signs of serotonin syndrome is confusion. You should be examining TD on patients who are prescribed stimulants.

Diagnostic: If none is conducted then discuss what labs you would be drawing to make sure the patient is safe. Pregnancy tests or urine drug screens may be warranted on patients. Hormone and vitamin levels may be considered. For example, some of the psychotropic meds can increase prolactin levels. Please support with evidence-based articles and guidelines.

25% Mental Status Exam (MSE) and Differentials

MSE – please read the Exemplar. This section you will describe the patient’s appearance, attitude, behavior, mood and affect, speech, thought processes, thought content, perceptions (hallucinations, pseudohallucinations, illusions, etc.)., cognition, insight, judgment, and SI/HI. See an example below. You will modify to include the specifics for your patient on the above elements—DO NOT just copy the example. You may use a preceptor’s way of organizing the information if the MSE is in paragraph form.

Differentials – please cite your work. You need to support your differentials with peer-reviewed articles.

Provide at least three evidence-based, peer-reviewed journal articles or evidenced-based guidelines that relate to this case to support your diagnostics and differential diagnoses. Be sure they are current (no more than 5 years old).

Books are not considered peer-reviewed despite books going through a rigorous review process.

5% for Introduction, purpose statement and conclusion. An introduction should be about 4-5 sentences discussing your assignment. Your purpose statement will be part of your introduction. Typically, I prefer my purpose statement to be the last sentence of my introduction stating, “The purpose of this paper is to…..”

You should have a separate reflection and conclusion per the rubric.
