PART 1-DEFINITION OF CHILD ABUSE AND NEGLECT USING YOUR STATE’S TERMINOLOGY

Accomplished- The summary fully defined child abuse and neglect using the state’s terminology.

PART 2-INFRASTRUCTURE

Accomplished- The survey fully articulated the age, nature, and condition of the community’s available housing, infrastructure needs such as roads, bridges, and streetlights; the presence or absence of functioning businesses and industrial facilities; the location, condition, and use of public spaces; the location and condition of the public buildings-the city or town hall, courthouse, and so on; the defined schools available along with their location and physical condition; and community resources related to child abuse and neglect, spousal abuse, or elder abuse.

PART 2-LEVEL OF ACTIVITY

Accomplished- The survey fully described the level of activity in the community, including the amount of activity on the streets at various times of the day, week, or year, the noise level in various parts of the community; the amount of movement of traffic at various times of the day; and the walkability of the community.

PART 2-AVAILABLE SERVICES AND RESOURCES

Accomplished- The survey fully explained the services and resources available to the community, including the presence, cost, and accessibility of public transportation; the presence of social centers, public libraries, parks, churches, community centers, and recreation centers; the availability of healthcare facilities, including how many there are, clinics and hospitals present, and accessibility of healthcare resources; resources for food and nutrition, including the presence of grocery stores and/or food deserts; and homeless services available to the homeless population.

PART 2-SAFETY SERVICES

Accomplished- The survey fully summarized the extent of police and fire services in the community.

PART 2-COMPILATION

Accomplished- The windshield survey data were fully appraised with a data overview of all community components as identified in the community TOOL BOX RESOURCE and the identification of strengths and weaknesses (gaps in service) of the community.

PART 2-NURSING AND FAMILY DIAGNOSIS

Accomplished- The survey data fully developed 1 family nursing diagnosis based upon an identified gap in community resources as an indicator of potential poor health outcomes and potential poor/weak parenting.
