Roseola Patient’s Soap Note

Hint 1 (Roseola) is the patient primary diagnosis.

A 2 years old girl was brought to the clinic with complaint of rash on her abdomen and spread out to other part of her body like the hands and legs according to the information obtained from the patient’s mother. Patient also had an elevated high-grade fever (103.3F) that started 3 days prior however cleared and resolved by itself a day ago. she know longer have an elevated fever however the rash started after the fever subsided. On the patient physical exam, there is a slight light pink rash noted on her trunk and abdomen, patient was also noted to develop a pink conjunctivae upon inspection. Patient also had a runny nose and non-productive cough for the past three days. The mother also stated that the toddler has not scratch or ichy the rash. Patient diet is normal, eating appropriately, she’s able to move her bowels and urinate normally as well.

Based on the Clinical assessment and physical findings which include a runny nose and unproductive cough with an elevated fever that subsided by itself then proceeded with rash two days later. Patient potential diagnoses is Roseola.

Chief Complaint:

Patient mother reports her 2 years old daughter condition with complaint of rash on her abdomen and spread out to other part of her body like the hands and legs according to the information obtained from the patient’s mother. Patient also had an elevated high-grade fever (103.3F) that started 3 days prior however cleared and resolved by itself a day ago.

Reliability and Source of History:

The patient is a 2-year-old baby. His mother is alert and oriented and able to answer most of the questions in regard to the baby condition.

Subjective and Objective Data.

Hint 1 (Roseola) is the patient primary diagnosis.

Other potential diagnoses for the Toddler are:

· Urythema infectiosum

· Upper respiratory infection

· Measles Rash

· Rubella Rash

(Each diagnosis should be supported with references)
