You learned Risk Assessment in OLS30000 or OLS33100, a pre-requisite for this course.

Part 1

Pick an area that you have control over - possibly your kitchen, garage, car, desk etc. and perform a risk analysis of the area.   Make changes to the area to mitigate the risks you found to make it safer. Submit this assignment as a formal Word Document. Include images of before and after. 

This report will be a risk assessment summary, not a full risk assessment with in-depth descriptions of code violations.  It should be a several paragraph summary with pictures describing 6 to 10 hazards in the area you chose and why they are considered risks. 

Part 2

After identifying and summarizing 6 to 10 hazards, develop a document or a table called  Summary of Risk Assessment  that highlights your findings in Part 1.

This document/table must include the following information:

a.    Hazards found 

b.    Initial risk level for each identified hazard,

c.    Control measure(s) taken to either eliminate such hazard or to reduce risk level for each identified hazard,

d.    Approximate costs for such control measure,

e.    Residual risk level after control was implemented for each identified hazard. 

This risk assessment summary will be a quick overview of those identified hazards, initial risk level, a control measure(s) taken, approximate costs of control measure(s), and residual risk level after control measure(s) was taken.

Format it like this.

1). Start with Introduction in one paragraph

2). Part 1: Provide details of all 6-10 hazards you noticed. Give a separate subheading for each hazard and under each hazard you should cover following main points:

· Explanation about hazard

· Initial Risk Level (The method about calculation of Initial Risk Level is given in presentation Risk Assessment) with justification

· Control measures taken to eliminate or reduce hazard. (Make changes to the area to mitigate the risks you found to make it safer)

· Approximate cost for control measure

· Residual Risk Level with justification

You are not required to have separate headings for above five things, instead you describe all five above things under the Heading for hazard. Explanation of above five things for each hazard would be about 150 words each. In part 1 you also need to include images before and after for hazards.

3). Part 2: This will be a table and table should have details about all 5 elements of each hazard which are listed in part 1 as well.

4). Conclusion in one paragraph.
