Week Two Resource List and Rationale: Use information from the case study in this week’s discussion to help inform your decisions. For each population listed below, you must briefly identify and describe the theory or model used to guide your decisions in this case, list the services/resources you have chosen, and provide a brief rationale for each. Your rationale should include the ethics and laws of your state. You must also identify your role as a counselor when providing these resources to your client(s) in the rationale.

Adult Mental Health Services/Resources for:

1. General

a. Inpatient

I called spectrum Weymouth inpatient and I spoke with Tatianna who is the clinical director there. Spectrum is a program for individuals from ages 18 and up. Spectrum is an adult treatment center for adult in need of services and mental health issues caused by trauma or past substance used. Spectrum is divided into three different programs inpatient, outpatient and the residential program called project turnabout. It is also branched out in different states. Spectrum provides services such as inpatient, outpatient to clients who are currently using substance used. Spectrum is known for its great services it provides to clients that come from different part of near by towns. Spectrum is open 7 days a week Monday through Friday providing services to client in need. The inpatient client is designed to service clients with substance use helping them with services such as counseling and 1:1 therapy session. They are also seen by a psychiatrist if necessary. Clients who attend the inpatient services are seen 3x a week by a clinician and are schedule to attend counseling along with 12 step programs to help with refraining from relapsing. After meeting with my client James, he was made aware of the importance of considering this program. Also, James was made aware that he will benefit from this program due to the services provided such as counseling and seeing a psychiatrist for any underline issues especially considering his recent marital status. Although he would greatly benefit from this program due to his past history of substance used according to the state, we as counselors cannot force individuals to participate in treatment. Although James may have a relationship with his previous wife as counselors according to the state, we cannot provide any information to his previous wife regarding his stay if he decided to do so. On a ethical stand point, although the clients safety is important Spectrum is not a locked down facility client can leave if they feel like they would like to pursue services elsewhere. Although spectrum provides counselors when clients decide to leave AMA (Against Medical Leave) a clinician will step in an educate client on their decision but are not allowed to block or force clients to remain in treatment against their own will. Due the laws in Massachusetts clients are to sign releases to provide information with a person of choice that a counselor can discuss their case with if they left treatment prior to completing and or court related such as probation or stipulated. As a counselor I will support my client and guide them in the right path. As a counselor it is my duty according to the ACA code of ethic to keep the client’s information confidential unless they allow me permission to share with a family member or loved one. According to the state of Massachusetts “Licensed Mental Health Counselors shall not communicate either verbally or in writing with others about a client without the client's express written consent, including any legal proceedings, except when the limits of confidentially may legally be invoked”.

iii. I contacted the Curahealth Hospital and I spoke with the director in charge of the mental health services program. Curahealth is a facility that is a lock down unit for individuals with mental health issues and psychiatric issues such as bulimia, binge eating, schizophrenia, bipolar and more. In this facility the use model is CBT and psychotherapy. Psychotherapy is used with clients to help them with a broad variety of mental illness and emotional difficulties. Especially in the facility dealing with clients who have used substance use who also have underlyning issues. Curahealth is divided into two department the first department is the psychiatric unit and clients normally stay for about 30 days. After each client complete their stay, they are assigned to a outpatient services for after care. During their stay they are seen by a case manager assigned to their case and a therapist. Each client has their own room and are encourages to attend mandatory groups on different topics related to their treatment. Clients are supervised 1:1 if necessary. Clients are also supervised by a charge nurse who provides medical care when needed. Curahealth is open 7 days a week Monday through Friday including weekends. Each therapist takes turns working weekend to ensure that a therapist is on cite if a crisis was to present itself. After talking to my client James, he was not interested in this facility due to the severity of differences. James was not comfortable with the idea of being in a lock down facility for 30 days. James stated that he feels that he does not need a 30-day lockdown care in order to prevent him from drinking. According to the state of Massachusetts a client has to be a threat to themselves and others in order to be put in a lockdown facility. James does not show any risk of hurting himself. On an ethical point James will need to agree to attend this treatment center. James cannot be forced to attend if he does not have any high risk of self-harm. Although he has a history of drinking excessively which could lead him to a high-risk James has to provide information and sign release in order to proceed further. It is my role as a counselor to support my client in any decision they have conclude on. As a counselor I will support my client and provide services that best fit and suit the help they need.

c. Outpatient

I contacted the outpatient services at New life counseling services, and I spoke with the head manager who oversees the mental health department. New life counseling wellness is a center for individuals with mental health services and looking to be seen by a therapist. Individuals are encouraged to attend at least three times a week their preferred time that suit them. Each client is seen by a professional LMHC in a private room where they discuss issues with their assigned counselor. Each counselor is mandated by the state of Massachusetts to maintain information form clients confidential. After each client feel like they reach a level where they no longer need a therapist, they are welcome back if they decided that they should be seen by one. “We are a multicultural organization that promotes the well-being of individuals, children, youths and families through high quality mental health and social services that are culturally competent such as counseling, support groups, advocacy, and education “(New life counseling wellness, 2021).

ii. [After speaking to my client as a counselor it is my duty to provide my client with resources that best suit him and his needs. James is a 45-year-old male who is currently going through a divorce and could possibly undergoing other underlining issues considered he has a history of substance use. This program is a voluntary program and clients are to attend on their own will. On an ethical point of view, clients are to sign a consent form if they wish to further share their experience with any loved one. According to the state of Massachusetts client’s information are to be confidential and not shared with anyone unless clients sign a release. Due to James state as a counselor I believe this would be a great after care for James to attend on the side as well to address any underlining issues that he is facing due to his past experience with substance use and marital status. Per the ACA guidelines as counselors it is our duty to protect and provide appropriate resources to clients.

2. Substance Use Disorder Services/Resources

a. Inpatient

i. I contacted the clinical director at Bedrock recovery center and spoke with Jesse who oversees the clinical department there. Bedrock recovery center is a treatment center that provides clinical and treatment services to clients who are currently experiencing hardship with substance use. Clients will attend groups and will be assigned to case managers and they will work with their case managers to complete any task that hinders them from completing treatment successful such as FMLA or Short-term disability. Case managers will help clients compete any unemployment or paperwork document to refrain from any distractions while in treatment. Bedrock is divided into two department clinical stabilization and detox treatment. Stabilization clients will complete a treatment plan and stay for a number of 30 days to ensure best care for clients. During treatment detox clients will attend groups every day and will be seen by a clinician that will help client to access consent and addressing nay issues. Client swill also be assigned a therapist and client will work with their therapist to complete paperwork including a release and insuring confidentiality while in treatment. While in both program the client will be provided with meals and encouragement to apply to employment if needed. Bedrock is also a 24 hours facility and clinicians are available every day.

ii. After meeting with my client James, I think this facility will benefit him due to his history of substance use. James presents in a stage of contemplation. If James agree to this treatment, he will be provided with great help to endure some sobriety time. After completing detox for seven days James can decide to attend the next program at bedrock for stabilization to continue his sobriety. James will also receive therapy 3x a week while in stabilization and will be seen every day while in detox side. If James decided to attend treatment according to the state of Massachusetts, he will be task to sign a release to inform the services. After completing both programs James will be provided with services for after care and outpatient in his nearby area. James will also be encouraged to work with case managers to schedule services as soon as his departure to refrain from distractions and relapse. As a counselor I will provide resources that best fit James and remain bias.

b. Partial Hospitalization

i. Andrew house is a treatment center that provides the services for substance use for clients who are in need of guidance to seek sobriety. Andrew house is run under Bay cove services and they provide services to individuals form age 18 years of age and up. Andrew house is open Monday to Friday. Andre house has nurses that provided medical care if needed After each client complete detox for 30 days they will be task to stabilization and will be seen by a clinician that will then began to learn skills needed such as, the ability to ask for help, and complete documents that are necessary to help the client remain in treatment. Each client will be provided with help from their counselor and worksheet to work on particular areas they are struggling in. at Andrew house clients will also be able to connect with others and possibly find a sponsor through connecting with others.

iii. After meeting with my client James, we discussed the differences of the resources he was provided. Due to his current state this outpatient will be a great fit for James to help seek further care. Considering Andrew house is also a dual diagnose it will help him with any underlining issues. James will also be seen by a clinician when he is in the first program and 3x a week when he goes to the stabilization program. James will also attend groups that will help him that cover all topics on substance use by staff who are well trained. As a counselor it is my duty to protect client form any harm. According to the state of Massachusetts if James agree with attending this program he will be provided with a release for consent and if he is to leave or any counselor need to contact his loved ones for accountability while he is in treatment.

c. Outpatient

i. [I contacted Highpoint Treatment Center and I spoke with the clinical director. Highpoint is a treatment center known for its outstanding services provided to clients that are in need of seeking long-term sobriety. Highpoint provides services such as inpatient services, outpatient services, residential services, and community-based services. Upon arrival clients will be screen then seen by a nurse for assessment then will be placed in one of their programs which is the detox part. In the detox part clients will be seen everyday by a clinician and will be assessed by nursing if necessary or in need of any medical attention. Each client upon arrival will meet with a clinician and complete a biopsychosocial, a treatment plan, risk assessment with their assigned clinician. The other program at high point is the stabilization program design for clients after completing detox rather at the facility or elsewhere. Clients are encouraged to complete 30 days there to ensure strong skills and develop coping skills prior to going out to the real world. while in that program clients will attend groups and be seen 3x a week by a clinician. clients will also meet with a care coordinator to explore different options for after care services after completing the program successfully.

iii. During session I met with James and we went over the release and consent forms. It was made clear to James the expectations of this program. If James would consider he would have to complete a serios of documentation including consents and release according to the state of Massachusetts a release must be acknowledge prior to clients proceeding with treatment. James will be a great fit for this treatment due to his substance use. James stated in session that “he has been drinking excessively due to his marital status”. As a counselor it is my duty according to the ACA code ethic that I ensure safety and refrain my client form any harm. in this treatment James will be provided with services and skills after attending group and will also be connected with alumni that will be sharing their experiences with substance use.

3. Services/Resources for Couples and Families

a. Tjocelyne counseling and consulting services provide services to family and is also empowering a positive path to help clients grow and for their wellbeing. Jocelyn is who I spoke with who is the LMFT at Tjocelyne counseling. There they provided professional services to family and marriage. Ideal client at Tjocelyne is a couple and married couple with unmet emotional. At Jocelyne clients will work with therapist to help them engage in successful systematic therapies. This counseling center is open Monday through Friday. This counseling center provides services from age 4 years old to 90 years old. At Tjocelyn they also provide psychotherapy for those in need. Tjocelyne is designed to help clients who seek help discover their true self, and current challenges. At tjolceyne some of the services provided are CBHI services, therapeutic groups, online video therapy. Clients have the ability to choose different therapist as they feel comfortable. Upon arrival clients will complete a screen and assessed to provide the best services.

James a 45-year-old male who is currently undergoing family conflict with is previous wife will benefit from attending Tjocelyn. After speaking with James regarding the different services such as family therapy and counseling for marriage he believed it would be a best fit for him. Prior to proceeding James will complete an assessment with a counselor and complete a release form. James will not be pressured to attend counseling this is a voluntary program. Due to James dealing with both mental health and issues within his marriage a marriage counseling could be beneficial for an after care. During this time James should focus on his sobriety. As a counselor I can inform him regarding my concerns, but it is entirely up to him on the directions he takes, and I will be there to support. According to the ACA code of ethics “Counselors facilitate client growth and development in ways that foster the interest and welfare of clients and promote formation of healthy relationships”. (ACA code of ethics, 2014).
