Methodology
Sampling/Setting
This research will be conducted in 150 patients aging 65 years or more admitted to Jackson Memorial Hospital Florida, with the confirmed diagnosis of Congestive Heart Failure to investigate the impact of a comprehensive teaching about the reduction of sodium consumption over the readmission rate because decompensating Cardiac Heart Failure. The Center is comprised of a Cardiology Division, a Cardiothoracic Surgery Division and the Arrhythmia Syncope Center. Among others this hospital has several services including Cardiac Catheterization, Percutaneous coronary angioplasty, cutting balloon angioplasty, coronary peripheral stenting, rotoblator atheretomy, electrophysiology, defibrillator and pacemaker implants, etc. As the hospital and the Heart Center bring together the collaborative efforts of highly skilled professional most of the patients have 65 years or more, and most of them came the hospital with cardiac issues.
Sampling Strategy
Convenience sampling will be used to include individuals selected according to the inclusion and exclusion criteria that reflects the population as identified in the problem statement. Consecutive admissions will be screened daily to identify eligible patients, who will require an admission diagnosis of HF made no longer than 3 months and being discharged home. These patients’ medical records will be reviewed to verify the patient’s diagnosis for HF has being made in a period no longer than 3 months. Patients excluded will be those with HF as a secondary diagnosis; patients with a HF diagnosis made longer than 3 months; patients with terminal illness in addition to HF; patients being discharged other place than home; and patients non able to give consent or without an identified proxy to give consent. The total sample will consist of 200 individuals to include in two groups to follow a research design.
Research design
The type of design to be used will be is a quantitative quasi experimental pretest posttest study to assess the effect of the proposed intervention on the rate of hospital readmissions in adults 65 years and older diagnosed with Congestive Heart Failure in JMH Hospital. A quantitative quasi experimental design will be used. Only one intervention will be applied in both groups; the intervention will be providing a comprehensive teaching about the reduction of sodium on diet to prevent decompensating Cardiac Heart Failure. This kind of study does not need a random assignment as the true experiment does need. A quantitative quasi-experimental research design, especially a one group pretest-posttest design will be used for this research. Gray et al, (2017) A pretest- posttest design was used in this research in both groups . The results confirmed that the a comprehensive teaching focus on reduction of salt consumption in patients with Congestive Heart Failure can impact positively on reducing the readmission rate because of decompensating Cardiac Heart Failure The design seems to flow from the research problem, theoretical framework, literature review and the hypothesis.
Quasi experimental studies are powerful methods to determine whether a cause-effect relation exists between the intervention and the outcome (Grove, Burns & Gray, 2013). Both two groups will be observed for 30 days and then a chart review will be conducted to see what the readmission rates are for the last 30 days and compare them.
This study may benefit from such design since and posttest evaluation will be applied to both groups and the trend will be followed prospectively and subsequent status evaluations with respect to an outcome can be conducted to determine which initial participants exposure characteristics are associated with it. As this study is conducted, outcome from participants in each group can be measured and relationships with specific characteristics determined that will answer the initial question (Song & Chung, 2010).
Extraneous Variables
Undesired (extraneous) variables will be controlled by using inclusion and exclusion criteria. Exclusion Criteria is the set of conditions that characterize some individuals which result in being excluded in the study (i.e. other health conditions, taking specific medications, etc.).. On the other hand, Inclusion Criteria are the set of conditions that studies must meet to be included in the review (or for individual studies the set of conditions that participants must meet to be included in the study; often comprises age, gender, disease type and status, etc.).
Inclusion Criteria: Patients with an admission diagnosis of Heart Failure made no longer than 6 months; patient is going to be discharged home. These patients’ medical records will be reviewed to verify the patient’s diagnosis for Heart Failure has being made in a period no longer than 6 months. Acceptance to participate in the study of patient/proxy.
Exclusion Criteria: Patients with Heart Failure diagnosis made longer than 6 months; admission because other causes other than cardiac heart failure , patients being discharged other place than home; and patients/proxy refused participated in the study.
