Group Formation (1 pages)
Values – List three guiding values for the group that summarize the shared purpose. Explain the importance of each of the values the group chose.
Roles – List the group roles that the group has defined for each group member. Provide a brief description for each of the identified roles.
Strengths – List 1-3 specific strengths that the group possesses. Note that strengths are resources, attributes or abilities that the group possesses that gives it an advantage.
Goals – List 1-3 stated criteria for the group’s success. Consider what it means for the group (and the individuals in the group) to be successful.
Work Process (1-2 pages)
Leadership – Describe how the group will organize the leadership role. For example, will the group have a designated leader or utilize a shared leadership approach? If a shared leadership approach is indicated, will a rotating leadership approach or divided leadership based upon areas of responsibility be used?
Accountability – Describe the planned process for group coordination, meetings, communication, and accountability. Consider how individuals will be held accountable for supporting the group’s success.
Decisions and Conflict – Describe how the group will make decisions and resolve conflicts and disagreements when differences arise. For example, will the group adopt a consensus model in which all group members must agree? Or will the group adhere to a democratic (voting) process in which the majority rules?
Risk Management – Identify 1-3 likely pitfalls and obstacles to success based upon prior experience in group projects. Descreibe in detail how the group can manage risks to avoid the identified pitfalls and overcome the identified obstacles.
Commitment – Conclude by directly stating the individual group members’ commitment to one another and to the shared success of the group.
