States has been vibrant in enacting policies that affect access and delivery of healthcare services

to the people. Major policies began with the establishment of the Affordable Care Act (ACA)

during the Obama administration. This federal program paved the way for other important

reforms to healthcare. These laws have been subject to different political wings that either add or

subtract from the reforms. Simmons (2019) , observed that Trump’s administration made

significant changes to the Affordable care act which includes the elimination of individual

mandates to register for premium ACA. This is an example of how policies about healthcare are

dynamic and subject to legislative influence by the federal government as well as influence by

political as political forces. The provision of healthcare is contingent on the availability of a

sufficient workforce capable of providing high-quality care to patients. The Affordable Care Act

has made a major contribution to addressing the issues that have arisen in the United States. It

has improved access to healthcare services.

Affordability and healthcare quality are two factors to consider. Despite these

improvements, healthcare practitioners have been in short supply, especially in rural areas. Since

Trump's American Health Care Act was signed into law, several bills have been introduced to

improve the quality of nursing in the United States.

The H.R728, the Nursing Workforce Reauthorization Act of 2019, is one such significant

bill that has been introduced. Congresswoman Tulsi Gabbard and Congressman Dave Joyce

introduced this bill. This bill would aid in the reauthorization of services as well as the recruiting

and retention of nurses (Dave, 2019). There is a nursing shortage, and the passage of this bill

would ensure that those interested in pursuing a nursing career have access to high-quality

education and training. This will vastly increase the quality of healthcare provided to patients.

This legislation would improve the delivery of nursing care and support the safety of American

nurses (Dave, 2019).

In rural areas, a shortage of nursing practitioners has hampered healthcare delivery.

Nurses will be able to provide individualized healthcare to patients in rural areas as the

workforce grows. This initiative provides targeted assistance to educational institutions that

prepare nurses to work in medically underserved areas (Dave, 2019). As a result, nurses'

educational standards can be raised from entry-level to graduate. Furthermore, when

organizations have enough staff, nurses can stay and there is no exhaustion or burnout.

To promote this proposed reform, nurses have duties and obligations. They have the

responsibility of identifying and advising those who want to pursue a career in nursing. To direct

nursing students, they must collaborate with high school counselors. Nurses welcome progress

and provide high-quality treatment because there are resources for education and training.

Quality Measures, Payment for Performance and Its Effect in Patient Outcomes

The Healthcare system is a dynamic field in most of its aspects of care delivery. Regarding

methods. Reimbursement for performance is a type of healthcare system that prioritizes

efficiency, patient satisfaction, and cost-effectiveness of services. These programs are intended

to boost production. The payment for results scheme encourages quality and overall value of

healthcare; outcomes for patient's wellbeing. This is because doctors and other health

practitioners are in high demand. They are devoted to providing high-quality healthcare. Also, in

such a system, incentives are provided to nurses and facilities if they perform well. Healthcare

providers are required to carry out a variety of developments to produce the best possible results

for patients (Mendelson et al.2017).

Roles and Expectations of Nurses

Payments for excellence have radically changed the way healthcare is delivered. They

have not only increased the quality of care, but they have also reduced healthcare costs. Nurses

are primary caregivers that play a critical role in providing high-quality healthcare. improving

the results of patients (Mendelson et al.2017). Furthermore, they have set goals and aspirations

for themselves in the functions that they are supposed to carry out to improve service quality.

With the support of pay for performance, they are recognized as significant contributors to the

implementation of healthcare initiatives. Nurses can access satisfaction ratings and safety reports

since they are available online, access and comprehend the data for their personal use.

Nurses are relied upon by organizations to identify possible threats and promote health.

These include detection of medical errors and correcting them promptly and appropriately

(Salmond & Echevarria, 2017). This is when payments for an organization's results are based on

customer satisfaction, the available facilities, and wellness outcomes. Nurses are required to have

new competencies to reduce costs and improve patient experiences.

Nursing Management and Leadership

New advances in modern healthcare delivery necessitate joint efforts to deal with

healthcare problems. As a result, the leadership models that are used are linked to a decrease in

productivity, reduced medical errors, better patient outcomes, and lower employee turnover

(Murray, 2017). Also, nurse leaders must supervise staff to ensure that they are not overworked.

Bullying and other types of abuse are prohibited. At all times, new leadership styles are needed

at all levels in a clinical environment. Leadership models that are transformational and cross-

cultural have been critical in holistically gathering for the patients’ needs. When it comes to

meeting the needs of patients the observed rapid changes in health care require a visionary and

capable leader. This is ensured by meeting the employee's well-being to facilitate healthy and

high-quality facilities. Cross-cultural leadership ensures that a nurse is mindful of world cultures

and how to care for people from different cultures. This is a great example of leadership that is

visionary as it looks at nursing as an all-inclusive scope that connects people of different

nationalities.

The leader may use transformational leadership to achieve his or her goals. Besides they

spend quality time teaching and coaching, as well as looking for possible solutions to the

problems. This is a good example of leadership in nursing care. Not only does this method

enhance healthcare quality, but it also aids in improved planning, coordination, and control. The

healthcare workers should be organized and mentored (Boamah, Laschinger, Wong & Clarke,

2018). In all clinical settings, the leaders should organize the personnel to meet the needs of the

patients. Technical advances necessitate the assistance of subordinates. As a result, leaders will

have professional assistance with them by instruction and coaching. The adoption of technology

in the workplace, particularly electronic medical records, has aided in the reduction of medical

errors.

Upcoming Trends

Nursing practice will become more relevant than ever in the coming years. Nursing

methods and responsibilities are evolving, and they will continue to do so. There have been

major advances in healthcare, but none that are particularly noteworthy. Technological advances

and a transition to community-based healthcare delivery are two major differences.

Technological Innovations

Remote control systems and high-tech patients are examples of technological advances.

In nursing practice, simulators and electronic health records are becoming more popular. To

provide quality care, technology can take over all operations in a clinical environment for

patients' care. This is because it has facilitated timely, individualized, and secure quality care.

New equipment and programs must be learned by nursing practitioners. It allows them to discuss

physiological parameters with the use of evidence-based practices and protocols to boost the

conditions of patients

Change in Healthcare Delivery to the Community

Since retail, nurse-led hospitals, and ambulatory centers have become more common,

nursing practice has changed. The future of healthcare delivery will be shaped by this trend of

treating patients outside of the hospital environment. It is tailored to the person and centered in

the community. As a result, nursing practice is moving toward home-based treatment and

engagement with patients (Husson University, 2016). Furthermore, they encourage preventive

treatment.
