Background

Diabet es is becoming a major problem in t he Unit ed St at es. Current ly, dat a from t he

Cent er for Disease Cont rol and Prevent ion indicat es t hat close t o120 million people in

t he US are suffering from diabet es or prediabet es. This number is very huge and

dealing wit h it is a mat t er of urgency. Wit h t he high number of diabet ic pat ient s and

t he number of t ime pat ient s spend in t he hospit al decreasing; home healt h care has

become a good opt ion for t he care of t hese pat ient s. Home healt h care provides

holist ic care t o pat ient s while improving healt h out comes. Many of t he individuals

suffering from diabet es combined wit h ot her condit ions oft en do not recognize t hat

diabet es should be t he focus of care, according t o t he American Diabet es

Associat ion (2018). This paper will focus on t he t opic of diabet es and t he effect s of

home healt h care on pat ient healt h out comes.

Problem St at ement

Wit h t he increasing problem of diabet es, more and more pat ient s are relying on home

healt h care t o help t hem in dealing wit h diabet es. Wit h t he huge number of people

dealing wit h diabet es, it is very import ant t o come up wit h ways t o improve pat ient

healt h out comes. In t he Unit ed St at es, more t han 100 million people are suffering

from diabet es or prediabet ic condit ions. Diabet es is a chronic condit ion which brings

wit h it ot her complicat ions, such as kidney failure. Therefore, it is import ant t o cont rol

t he glucose levels of pat ient s wit h t hese condit ions. Wit h t he huge number of people

suffering from diabet es and managed t reat ment requiring consist ency t hat cannot be

achieved in one hospit al visit , finding a solut ion t o improve t he healt h out comes for

t he diabet ic pat ient is crit ical. Addit ionally, t here are a large number of diabet ic

pat ient s who are above 65 years of age. In fact , about 42 percent of diabet es

pat ient s in t he Unit ed St at es are above t he age of 65 years (American Diabet es

Purpose of t he change proposal

The principal purpose regarding change proposal is t he ident ificat ion and

implement at ion of a healt hcare plan t hat could help in t he improvement of diabet es

management in home healt hcare. The purpose of home care for pat ient s wit h

diabet es is t o improve t heir healt h out comes by consist ent ly adopt ing various

int ervent ions such as medicat ions, doing rout ine physical exercises, eat ing t he proper

diet , et c. as recommended by t he healt h professionals. Again, t he change proposal

aims at providing solut ions on how home healt hcare can be successful bot h t o t he

hospit als t hat could have ot herwise admit t ed t he pat ient s and t he pat ient s

t hemselves t hrough collaborat ion wit h home care nurses. Since diabet es is a pressing

concern in America and hospit als have been overburdened by a large number of

diabet ic pat ient s, improving home healt hcare is a priorit y (CDC, 2017). For t his reason,

t he change proposal aims at giving an insight int o some of t he improvement s t hat

could be act ualized in order t o improve t he qualit y of healt h out comes for diabet ic

pat ient s.

PICOT

The PICOT quest ion t o be used in t his assignment is in pat ient s of 65 years and

above, how does t he use of t he home healt h care syst em compared t o clinical or

inst it ut ional healt h care syst ems enable t hem t o live a much bet t er life in t heir old

age? The primary focus of t he st udy is based on t he demographics of old or aging

populat ions who have been out lined as people at higher risks of diabet es. The

research analysis present s t he aging populat ion as t he primary populat ion considering

t hat diabet es primarily affect s people in t heir senior years. For t his part icular group of

pat ient s, t he int ervent ion is solely based on improving out comes and maint aining a

comfort able lifest yle for t he out lined pat ient s at high risk. The int ervent ion is based

on t he pat ient s receiving healt h care in t heir homes, an environment t hat t hey are

familiar wit h.

It is not able t hat t he majorit y of t he pat ient s are immobile due t o degenerat ive

issues and condit ions. It is t herefore imperat ive t hat a support ive environment ,

t hrough formal caregiving, is inst it ut ed t o facilit at e t he care of such pat ient s. The

result s or out comes of an int egrat ed home healt h care program for diabet es pat ient s

prompt ed healt h care facilit ies t o consider t heir choices wit h regards t o t he pat ient 's

abilit ies and informal care availabilit y in improving t he out comes of diabet es pat ient s.

Lit erat ure search st rat egy employed

The st rat egy employed for searching lit erat ure wit h regard t o t he management of

diabet es in home healt hcare incorporat es t he use of credible and qualit y sources of

informat ion t hat specifically relat es t o t he issue at hand. Therefore, t he lit erat ure

used has been gat hered from peer-reviewed journal art icles, credible websit es of

healt h-relat ed organizat ions, books, newspapers, magazines, and diabet es-relat ed

report s. Also, t he bibliography sect ions of art icles and report s were also ut ilized in

gat hering more credible informat ion. To obt ain t he finest det ails wit h regard t o t he

t opic under discussion, t he researcher also incorporat ed t he use of keywords and

t erms t hat pert ain t o diabet es and home healt hcare. In order t o obt ain recent

informat ion, t he search was limit ed t o t he last five years.

Evaluat ion of t he lit erat ure

First of all, t he informat ion used in t his discussion is very relevant t o t he t opic at hand

because it is gat hered from credible and qualit y sources. Secondly, most current

lit erat ure has been obt ained by using sources published over t he last five years.

Thirdly, t he sources ut ilized t o provide specific informat ion t hat is very specific wit h

regard t o addressing bot h t he problem st at ement and t he research quest ion. Again,

t he available lit erat ure on t his t opic is fact ual since it is based on scient ific research

based on evidence and real case st udies regarding diabet es and home healt hcare.

Nursing Theory Applied

Self-management t heory explains how self-efficacy can be enhanced in improving

pat ient healt h out comes especially for pat ient s suffering from chronic condit ions like

diabet es. Essent ially, healt hy behaviors like successful sympt om self-management

have t he pot ent ial of reducing suffering from diabet es (CDC, 2017). Here, home care

Proposed Implement at ion Plan and Out come Measures

Implement at ion plan incorporat es t he provision of diabet es-relat ed educat ion t o t he

pat ient and/or t heir family wit h regard t o prevent ion and management of diabet es;

administ rat ion of insulin, blood sugar t est s, et c. It would also incorporat e t he provision

of specialized services t o t he pat ient by home care nurse in t he home set t ing as well

as providing t he pat ient wit h necessary equipment and resources t o perform specific

t asks (American Diabet es Associat ion, 2019). Out come measures would be relat ed

management of sympt oms including; reduct ion of weight , reduct ion in fat igue, normal

urinat ion, improvement in vision, et c.

Implement at ion Barriers and Ways of overcoming t hem

One of t he barriers t o implement at ion of t he above plan is non-compliance by t he

pat ient t o t he list of recommended act ivit ies. This can be overcome by let t ing t he

pat ient underst and t he repercussions of non-compliance. Anot her barrier is t he lack

of support from t he family members. Here, t he family members can be educat ed on

how t o provide a comfort able environment for t he pat ient . Moreover, scarcit y or

inadequacy of home care nurses may also hinder t he implement at ion process. The

pat ient can seek t he services of privat e nurses depending on t heir financial well-

being. In addit ion, diabet es is a chronic condit ion t hat requires huge cost s for it s

management ; t herefore, t he implement at ion could also be halt ed by high healt hcare

cost s. These cost s can be avoided by seeking government -sponsored healt hcare

programs like Medicaid or Medicare (CDC, 2017).

Conclusion

In conclusion, home healt h care is becoming very import ant when it comes t o dealing

wit h t he huge burden of diabet es wit hin t he Unit ed St at es. However, wit h t he

uncert aint y of it s beneficial effect s, it is essent ial t o ensure t hat bet t er healt h

services are provided t hrough home healt h care. When implement ed well, it can

improve t he pat ient s’ qualit y of life, healt h out comes, and st abilit y of glucose levels.

To improve knowledge concerning t his t opic, it is essent ial for fut ure researchers t o

look at how home healt h care can impact pat ient s living wit h diabet es.
