Report of Suspected Child Abuse Form (20 points) Due October 25th by 11:00pm in D2L

This assignment is due online in D2L on Friday, October 25th by 11:00pm. This assignment will be graded

based on how well you complete the form (details matters)

1) Go to: http://www.pa-fsa.org/Portals/0/Docs/Mandated-Reporters/CY47%20Final.pdf

2) Complete the form thoroughly based on one of the cases we have discussed in class. You may need to

make up contact information for the child, family members, and perpetrator. Your responses need to be

detailed and specific. Approach this as if you were reporting on a real case.

3) For sections where you are required to give a lot of description (Section 6 and 8A-E), you will need to

submit your answers in a separate file. If your writing goes beyond the size of the box, it will not be

viewable once you save it.

4) Save Form CY47 and your accompanying Word document. Submit both files to the appropriate D2L

folder by 11:59pm on Friday, March 29th.

i. Note that some people have had issues saving the completed form. If this is the case, you need

to submit the form in hard copy and your extended answers in a Word Document in D2L.

5) Failure to complete the form with detailed and concrete details and/or answering in complete

sentences, will result in the assignment being graded down.

Important Notes!!!!

***You can only complete the form for a case we have already talked about in class. If you do it on another case, you will

not receive credit!

***The online form does not always save. This varies from computer to computer, web browser, etc. I cannot fix this, so

you will need to make sure you are submitting a completed form that I can read (if the words bleed over, then I cannot

read it to grade it). Blank documents will not be given credit.

*** If you are worried about the document issues or just, don’t want to deal with them, you can submit your paper in

hard copy to my mailbox in the Sociology Department. If you go with this option, your paper must be in by 5pm on

October 25th. Papers submitted after 5pm will not be given credit.
