I work in an inpatient psychiatric hospital. In California, the four main types of payers are: private (ex. employer-based), Medicare, Medi-Cal (California’s state Medicaid program), or uninsured (no payer). College Hospital recently opened a new crisis stabilization unit that is funded by the county.  The Board of Supervisors approved a three-year, $13.3-million contract for the crisis stabilization center at College Hospital, an acute care facility at 301 Victoria St. (Money, 2019). Board Chairwoman Lisa Bartlett stated, “This additional crisis stabilization unit will prove to be an asset for those individuals and their families during their time of greatest need,” (Money, 2019). The funding that is allocated for this center will help patients access psychiatric assessment, crisis intervention, therapy, medication, and other services they require.

Efficiency and productivity in nursing are highly impacted by the nurses’ fatigue. One area of difficulty encountered is the stress that comes with dealing with patients in a nursing and rehabilitation center. Trinkoff et al. (2008, p. 473) concur that a healthy and well-rested nurse is critical in empathetic administering vigorous advocacy, patient care, and vigilant monitoring. The challenge came with the patients' continuous trickling, though the nurses are few. It is the duty of a nurse to ensure all patients receive the necessary care. However, through interaction with various nurses, I learned that there was a recess room. Nurses, upon making a mutual agreement with other nurses, two or more nurses can take time off, rest and freshen up for a few agreed hours, and then switch with others in need of the service. This is an intervention to prevent burning out during working hours since rest is paramount for efficiency. 

Projects in the area of work are funded by the civil money penalty funds (CMP). It is a Centers for Medicare & Medicaid Services (CMS) monetary penalty imposed against nursing homes for non-compliance. The funds are then reinvested into improving nursing facility initiatives and projects supporting family and residential councils (Centers for Medicare & Medicaid Services, 2020). The CMP is regulated by the Affordable Care Act of 2010, which promotes health facility improvement through varied projects supporting the provision of quality healthcare.  According to the Centers for Medicare (2020), various projects such as surveyors and staff joint training and technical assistance training forums for programs contribute to performance improvement and quality assurance. Therefore, by following the right procedures in the application of the funds and ensuring that the necessary requirements are fulfilled, various projects in the working site have been implemented, and quality care is amongst the essential requirements from nurses in the facility.
