Now that you have learned how to build a survey using Qualtrics, it is time to put your skills to
work and create your own web based questionnaire.
For this exercise, you will be building a short questionnaire for the University of Florida
Athletics Association. The Athletics Association would like to create a short survey to assess the
quality of their concession vendors at Ben Hill Griffin Stadium. The survey will be sent to a
random sample of students that attend the UF Homecoming game.
Before you start developing the survey, you need to identify how many individuals should
receive an email invitation to participate in the survey. Let’s assume that 25,000 student tickets
were sold for the game and you expect to receive a 25% response rate (due to response error
like incorrect email account information and because many students will choose to not
respond).
Hint: If you plan to survey 1,000 you will only receive 250 usable surveys. To generalize to
25,000 students, how many usable responses do you need? Multiply that number by 25% to
identify the number of students that will be in the sampling frame.
Follow these steps to create your questionnaire:
1. Open Qualtrics at elearning.ufl.edu 2. Log in using your Gatorlink information
3. Create a new survey through the Quick Survey Builder
4. Name your survey Student Football Survey – Your Last Name
a. Hint: Use you real last name for the Your Last Name portion of the title
5. Add the questions below in Multiple Choice Format

Did you attend the UF football game against TEAM this year?

Did you purchase any food or drinks inside the Stadium during the game?

6. Next, the Athletics Association would like to know which concessionaire(s) the student
tried during their time at the game.
Add the following question in multiple choice format and make sure you allow for multiple
answers to be selected.
Which concessionaire vendors did you purchase from during the game?
- Domino’s Pizza
- Deli Subs
- Quesadillas
- Fresh Whole Fruit
- Coach Mac Dog and Bratwurst
- Other
Add a text entry box to the Other answer choice so respondents can type in another vendor
that was not listed.
7. Based on recent feedback that was collected by unsolicited emails to the UAA, a lot of
negative feedback has been expressed about the quality of service received from their
vendors. Add the following question, formatted as “Text Entry” but change the text box
size to “essay”.
Do you have any comments or suggestions on how to improve the quality of the service you
received during your encounter with any of the vendors at Ben Hill Griffin Stadium?
8. You now realize that the question you created in Step 6 above should only be delivered to
those that selected “Yes” to this question: Did you purchase any food or drinks inside the
Stadium during the Vanderbilt game?
Therefore, add Skip Logic to this question so that respondents who choose “No” are skipped to the
end of the survey. Only those that answer “Yes” to this question should be eligible to respond to the rest
of the questionnaire regarding the quality of their experience with specific vendors.
9. After looking over your survey, you decide that you would like to add a short introduction at the
beginning of the survey to give some brief instructions.
Add a question to the beginning of your survey and change the question type to “Text/Graphic”. Once
you have done this, enter the following text in the text box:
Thank you for participating in this questionnaire on the quality of your experience at Ben
Hill Griffin Stadium. We value your opinion and appreciate your honest feedback.
This survey is being sent to ### students that attended the game and will take just 2-3
minutes to complete. Please click the >> button below to continue.
For the ### in these instructions, insert the number of respondents that you determined you needed to
send this survey to represent the 25,000 students that attended the game.
10. Add a page break after your introduction so that respondents see your instructions on one
page.
11. Add emphasis to your introduction, by making the words “value your opinion and appreciate your honest” appear in bold. Hint: Use the Rich Content Editor for the question.
12. Open the Look and Feel option and choose a skin from the menu. If you like the current look of the survey, just keep the existing UF default skin.
13. Open the Survey Options option. Add a back button to your survey.
14. Launch the survey by clicking on the Distributions tab for a link. Get a single resuable link to paste later.
15. Copy and paste the survey URL to a Word document. You will submit this URL address in the
Assignments tab for this assignment. The live Questionnaire linked to your URL will be graded for
accuracy.
Your final survey should look something like this:
