Selected Hospital

In this section, you present your team’s selected hospital here. The requirement asks you to describe the nature of the business, services or products, and customers served by your chosen hospital. You do not need to write several paragraphs for this requirement. A half page is enough. Be sure to cite your selected hospital’s website to support your statements.

Quality Improvement Measure

In this section, you evaluate one measure of your choice. For instance, the emergency department at ABC Health, sees over 5,000 patients a year (Medicare.gov, 2022). The median time patients were in the emergency department before being discharged was 188 minutes which is over the national average of 175 minutes (Medicare.gov, 2022). Remember that each team member must have a unique measure.

Importance of Measure

In this section, you discuss the importance of your selected measure (e.g., accreditation status, patient safety, and/or financial status of the hospital). Be sure to cite scholarly sources to support your arguments.

SMART Goals

In this section, you define two to three SMART goals for your selected measure to improve. Be sure to include all five elements—specific, measurable, attainable, relevant, and time-bound—in each goal. Here are some examples of SMART goals:

· To hire and train additional five nurses to ensure appropriate staff-to-patient ratios during peak times by 10/31/2022.

· To implement an up-and-running emergency department tracking system by 12/31/2022.

· To ensure that 95% of the clinicians such as physicians and nurses have completed sepsis care training to improve identification and treatment by 10/31/2022.

· To reduce the turnaround time @ 10% for laboratory testing for lactic acid/lactate levels for sepsis by implementing point-of-care testing at the bedside by 12/31/2022.

Policies to Improve Measures

In this section, you analyze specific local, state, or national policies (e.g., The Joint Commission Standards) that have been developed to improve your selected measure based on evidence-based practice research. Be sure to cite scholarly sources to support your arguments. The requirement asks for a 3- to 5-page paper. If you complete your W2A for more than 5 pages, you need to double check your paper because you might not follow the requirements properly.

Conclusion

Your conclusion, in just one paragraph, should be a summary of your essay and should repeat the main ideas.  Do not introduce new ideas in Conclusion. You don’t need to include any in-text citations in introduction and conclusion.
