Quality Improvement

The facility should react to the competing crisis event by immediately hiring crisis nurses to help in reducing the nurse-patient ratio. From the scenario, it is evident that one nurse has to serve many patients. Hiring crisis nurses is, therefore, a system-based solution in which a balance in the ration is attained. Such a response will maintain quality care outcomes such as safeguarding and enhancement of experiences of patients. For instance, a nurse at a “sharp-end” risk the lives of patients attached to him or her in the healthcare unit since the competing crisis compromises a nurse’s ability to comply with patient safety measures.
Adequate nurses in the unit reduce the chances of a process failure and undue pressure while increasing the existing quality process when addressing and finding a solution to crisis events. This reduction prevents the occurrences of a competing crisis. This solution to a clinical staff shortage remains as an approach to improve care quality. A quick response to the crisis through the system-based solution is that nursing has, for a long time, remained as a vital factor in establishing the care quality in hospitals and the nature of patient outcomes. With adequate nursing staff, charge nurse and nursing supervisor can effectively achieve coordinated and controlled supervision across the facility, since a nurse assigned to patients on the unit will have time to communicate to their seniors in case of such competing crisis events. Work pressure, resulting from a clinal staff shortage is the only factor that makes nurses not to report the challenging situation but is best addressed through hiring crisis nurses.
The best approach to end lengthy waits and congestion is the implementation of the Emergency Department Information System (EDIS) to ensure the clearance of patients visiting and discharged is automated. The automation will mean that patients' records are electronically stored and retrieved very fast, unlike the use of manual records to enter and retrieve patient information (Greiner et al., 2018). The EDIS will help in improving the quality of care services provided within the emergence department as well as meeting and satisfying the care needs of patients.

To begin with, I would say that the charge nurse blamed the individual nurse for not monitoring the patient, but it was not her mistake that she was given multiple complication cases to deal with. The nurses should have had a better system-based solution to prevent these kinds of crisis to happen. Accusing the nurse for a fault was not an ethical way for the charge nurse. The purpose of having a charge nurse is to keep the unit discipline and organized. Even though the nurse did not ask for help, they should have had a report of what patients does each nurse on the unit have, and what is there medical situations. One system-based solution that can be set to eliminate like this situation is that the charge nurse should have a list of the current admitted critical situation patients and have them on close monitoring. These critical situations should not be given to a busy nurse who have another 3-6 patients. There is no doubt that the nurse is usually busy passing medications and taking care of other patients, so giving her one or two critical situations that need immediate monitoring is difficult to manage and keep an eye on. Critical situations require a nurse who is not busy taking care of other patients. Another system-based solution that I think would work is to have a CAN or PCT to perform the hourly rounding to check on patients while the nurse is busy with other patients. By doing the hourly rounding by the CNA or PCT, the nurse will get an update about the patient’s status if new findings were found.
