Overview: In this class you will create a test report based on provided information. There is not time to obtain and use actual tests in this 8-week format, so you will integrate provided information from a real client. In addition to the discussion board posts, you will complete these additional assignments:

Assignment 2: This is a brief multiple-choice quiz, reviewing statistical concepts that you need to know to competently use psychological tests. It does not count toward your grade, but it is important to use it as a guide to determine what concepts you need to review. The quiz is located under additional resources. Assignment 4: This is a brief essay to help assess your writing skills and use of APA format. Assignment 6: This is a brief essay to review ethical and practical aspects of testing. Assignment 7: Interview a “client” of your own choosing to create a psychosocial history and mental status exam report. Use “Interview Guide.doc” and “MSE-1.rtf” to collect the information. Write it up in the format of the Sample Report in additional resources. You will not need to include the testing section of the report for this assignment, as you will not be conducting any psychological testing. However, you will need to include the identifying information, reason for referral, psychosocial history, MSE, diagnostic impression, and summary/recommendations. Assignments 9, 12, and 14: Each of these assignments requires you to interpret a test protocol, found in additional resources. Each week you will build your report by adding an additional test (WAIS-IV, WRAT-4 and MMPI-2). Use “Frank’s” information for the history and mental status sections of your report. These are raw materials to incorporate into your report. If you feel some points need elaboration or clarification feel free to do an imaginary interview with Frank and incorporate additional information. Each report should follow the format of the Sample Report in additional resources, and will include referral information, history, mental status, test interpretation, diagnostic impressions, and summary/recommendations. Also refer to the Psych Report Writing in additional resources as a helpful guideline in writing about each of the test results.

Assignment 18: This final assignment requires you to submit a final test report, incorporating all of the feedback that you received on previous assignments. You will need to add the PAI interpretation to the final report as well as the WHODAS 2.0. Please see the WHODAS form under additional resources; use this blank form to generate Frank’s WHODAS score per the history given and provide a brief paragraph description of Frank’s presentation in the final report. Materials in Additional Resources: Mental Status Exam (MSE-1.rtf): This is a form for conducting a mental status exam. The

exam should be conducted as an interview and your observations and responses to questions recorded on this form. The information collected can then be incorporated into a mental status exam report, as a narrative with headings. Interview Guide (Interview Guide.doc): This form can be completed by the client to construct a psychosocial history. You should review and clarify responses by interviewing the client. The information collected can then be incorporated into a psychosocial history report, as a narrative with headings. Psychological Test Report Writing (Psychological Test Report Writing.docx): This document provides important guidelines for writing a psychological test report, as well as specific information about interpreting the WAIS-IV, WRAT4, MMPI-2, and PAI. At the end of this document you will find a table to convert scores to percentiles and classifications. Sample Report (SampleReport.docx): This is a psychological testing report, reflecting the assessment of an actual client. Use this report as a guide for the format and style of your own report. Psychological Testing PowerPoint (PsychologicalTesting.pptx): This is a PowerPoint presentation that reviews psychometric and statistical concepts. The presentation doesn’t assume any statistical background and is important to review. There are individual PowerPoints to assist with the interpretation of each test. Review these as you write your interpretation of each test: WAIS-IV.ppt, WRAT4.ppt, MMPI-2.ppt, and PAI.ppt. Frank Psychosocial and MSE (Psychosocial.pdf): This form gives the psychosocial history and MSE for “Frank,” an actual client, with identifying information removed. The test protocols are also Frank’s. You will write up this information in narrative format, with headings, and combine this information with Frank’s test protocols, to create a comprehensive psychological assessment report. WAIS-IV and WRAT4 Protocols: The WAIS-IV protocol is to be used to interpret for assignment 9 and the WRAT4 protocol is to be used to interpret for assignment 12. MMPI & PAI Protocols: The MMPI-2 protocol is to be used to interpret for assignment 14. Estimate the T-score for each scale from the graphed line (T-score>= 65 is a significant elevation). Interpret the PAI protocol in your final report (assignment 18) – T-score >= 65 is a significant elevation. WHODAS 2.0 Form: This enclosed form is blank in which you need to fill out to generate Frank’s WHODAS score based on the history provided. Then, provide a brief discussion of Frank’s presentation from the form in the final report.
