Information on your Client

When you are ready to start this assignment, you will need to utilize an online random client generator to retrieve the details specific to your client. This link will be provided for you in the course and is available here: Client Generator-o-Matic.

When you visit the Client Generator-o-Matic you will be given a description of the client you are going to do your assignment on. You can press the “Randomize” button as many times as you want to configure your client.

The Client Generator-o-Matic provides you with a paragraph including the following information:

· 12-year-old child’s name (you need to decide what sex/gender the child is)

· Diagnosis

· City/State in which the child is moving to.

· The child’s favorite hobby.

· The environment in the school where the child has a history of struggles.

You will build this assignment based on these details about your client. Here is what the output looks like.

Assessment Scenario

For this assignment your role is a community-based social-worker. You have been assigned to your client because their family is moving to a new area in the United States. It is your task to help with advanced planning to assure the continued success of the child in the new environment. The document you are to produce is what you will provide to the family prior to their move. Below you are going to see very specific instructions about what this document should address. There is also room for creativity in this assignment. The family’s last name, occupations, lifestyles, etc. can be added to enrich the “backstory” that goes along with your assignment.

Each of the sections outlined below should be formatted with bold headings. Keep narratives short and use bullets when appropriate. This document should be neat and professional looking reflecting the professionalism of your work.

This page represents the cover of the document you would be giving to the family. You can be creative about this, and it should contain the name of the child, a title for the document, your name (as the social worker), the date, and information about your “agency”.

Submission Artifact 3: Back Story – CLO 1

You will begin your case study with the story of the 12-year-old. This is also a creative process. Based on the information you were provided, create a story up until this time in their life. This section should only be about a page long. At the end of this section, the reader should understand your client as a person, their character and personality, and their perspective on this move. This section should also summarize how their diagnosis has impacted their lives. Past successes and challenges should be outlined here.

Submission Artifact 4: Diagnostic Criteria – CLO 3

When you receive your case study, the child’s diagnosis is going to be ONE of the following:

· Anxiety

· Depression

· Oppositional/Defiant Disorder (ODD)

· Conduct Disorder (CD)

· Obsessive-Compulsive Disorder (OCD)

· Attention Deficit – Hyperactivity Disorder (ADHD)

· Autism Spectrum Disorder (ASD)

· Tourette Syndrome

To document the diagnostic criteria for your child’s diagnosis you are going to visit the CDC page on Children's Mental Health.

In this section you will include the following:

· The name of the diagnosis.

· Create a bullet list of the criteria as stated in the CDC website on Children’s Mental Health. (Cite the source of this information in your paper by copying the URL and including it in this section).

· Create another bullet list indicating which criteria your client meets. Provide a brief description of how their behavior meets these criteria.

Submission Artifact 5: Etiology – CLO 5

Etiology refers to the “causes” of the disorder. In this section you are going to describe research that has been done related to the causes of this disorder in general and then outline specific aspects of the history of your client that contributed to this diagnosis.

Part 1

Conduct brief research on the generic etiology of this diagnosis. This should be generic, not specific to your child’s situation. Specifics related to your client should not be in this part. Your evidence-based etiology should describe the following factors associated with the diagnosis.

· Biological Factors

· Psychological Factors

· Social Factors

Part 2

Gather evidence on the clinical prevalence of this disorder among 12-year-olds.

Part 3

Outline (creatively) the history of the client and their family that aligns with the etiology of this disorder.

Be sure to use in-text citations to the sources that you use and make sure you include ALL sources in the “References” section at the end of your paper.

Submission Artifact 6: Diagnostic Assessment – CLO 4

Identify three clinical instruments (aside from clinical observations, interviews with teachers, etc.). Find three instruments that would be used by a Clinical or School psychologist to test for the diagnosis. Descriptions for each test should include the following

· The name of the test in bold text

· A brief description of the test and how it is used to diagnose.

· A link to the actual test itself and/or it’s. publisher. Do not link to articles that describe the test…I want you to find the actual source of the test.

The sources of these actual tests should be included in your “References” section.

Submission Artifact 7: Treatment – CLO 6

This section will focus on symptom-reducing interventions based upon the medical model.

Medication

Medication is often a first line approach to symptom reduction.

· This section should cover at least two different medications commonly used in the treatment of this disorder.

· You must provide a reference to where you obtained this information.

· For each medication you should describe the following:

· Name of the medication

· Typical dosage for a 12-year-old child

· Potential side effects.

Complementary Treatments

In addition to medication, other interventions to address symptoms may also be included in the overall plan. Based on your research, select two of the following (one MUST be nutrition). You must select interventions that have been shown to be effective on reducing symptoms with children with the diagnosis.

· Psychotherapy (including modalities such a CBT, Play Therapy, Dialectical Behavior Therapy, and Family Therapy)

· Allied health therapies such as Occupational, Physical, and Speech Therapy

· Applied Behavior Analysis

· Nutrition

· Exercise and Fitness

For each complementary treatment that you select, you must describe the application of the therapy to your child and include a reference to a peer-reviewed article related to that modality and its effectiveness with children with this diagnosis.

These sources should be supported with in-text citations and in your “References” page.

Submission Artifact 8: Rehabilitation – CLO 6

While “treatment” focuses on efforts to reduce symptoms in order to restore functioning for the client, rehabilitation focuses on modifications to the environment in order to help the person with the disorder thrive in the here and now. It assumes that symptoms may be present and relies upon plans to account for they may impact the client in that specific environment. In rehabilitation the assumption is that the environment needs to change, not the client.

In this section you are going to focus on the hobby and environment in school that were specified for your case study.

Hobby Rehabilitation

Your client has a hobby. It will be important for you to locate specific resources in their new location to assist them in maintaining this hobby. Those resources may be at the school or in the community. They may even be at a distance or online. Either way, your work is to establish a plan for the client to continue these activities in their new home. Include the following in this section:

· The name of the hobby.

· Locate specific resources in their new location that will support this hobby.

· Include the names of individuals or business.

· Contact information.

· Addresses to locations.

· Any websites or other resources specific to this location.

· Based on the resources available in the area you will now create a plan on how to address any barriers your client may have.

Barrier Plans

Barrier Plans identify specific barriers that are presented when a person with a disability encounters a specific environment. They are specific to the client’s symptoms, the features of the environment, and resources available.

Your plan will list both symptom-based and pragmatic barriers and potential medication side effect barriers. Symptom-based barriers are those that arise when symptoms may make it difficult for the person to thrive in that environment or create challenges to environments to accommodate them. Pragmatic barriers are those that revolve around issues such as cost, resources, transportation, etc. Take a look at the side effects of the medications you have listed and identify any potential barriers associated with medication side effects.

For EACH barrier you will come up with a solution. That solution may entail physical changes to the environment, psychoeducational teaching of the people in the environment, response and emergency plans, alternate ways for the client to interact, motivation interventions, etc. This section should be formatted as follows:

· Identified Symptom Barrier #1 – Description of the actual barrier and inhibits full participation.

· Specific Rehabilitative Intervention that could be put in place to address the barrier

· Identified Symptom Barrier #2 – Description of the actual barrier and inhibits full participation.

· Specific Rehabilitative Intervention that could be put in place to address the barrier

· Identified Pragmatic Barrier #1 – Description of the actual barrier and inhibits full participation.

· Specific Rehabilitative Intervention that could be put in place to address the barrier

· Identified Pragmatic Barrier #2 – Description of the actual barrier and inhibits full participation.

· Specific Rehabilitative Intervention that could be put in place to address the barrier

· Identified Medication Side Effect Barrier #1 – Description of the actual barrier and inhibits full participation.

· Specific Rehabilitative Intervention that could be put in place to address the barrier

· Identified Medication Side Effect Barrier #2 – Description of the actual barrier and inhibits full participation.

· Specific Rehabilitative Intervention that could be put in place to address the barrier

The number of barriers should be based on real-world expectations. Barriers and interventions should be specific, well-defined, and substantive enough to allow for real-world impact.

School Rehabilitation

You will approach this section in a very similar way in which you approached the Hobby. This one, however, will involve the school. Your case study identified a specific environment in which your client has struggled. You will focus on that environment for this section. Include the following:

· The name of the actual school they will be attending in their new location (pick one if they are moving to a large city location).

· Specific contact information related to accommodations within the school.

· Specific contact information to the Department or classroom that is indicated by your case study (if applicable)

· Name the environment.

Similar to the list of barriers and interventions for hobbies, make a list of barriers and interventions for the environment. Format these in the same manner.

Rehabilitation plans are often very creative. For instance, a child that exhibits complex tics related to Tourette’s Syndrome may benefit from an in-service to teachers and students to help them understand the symptoms, a change in policies of the school regarding outbursts in class, and a contact person for the student with the tics to go to if they experience any bullying or rejection from their peers.

Notice how the rehabilitation plan assumes that the symptoms will persist and that a plan be made to deal with it.

Remember what we have learned. It is clear to see that different symptoms among different people will create different barriers to different activities in different situations!

Submission Artifact 9: Treatment and Rehabilitation – CLO 2

In this section you are going to write an essay comparing and contrasting the treatment and rehabilitation approaches. Your essay should identify ways in which they are similar and ways in which they are different. Outline their strengths and weaknesses with specific reference to the ones included in your case study.

The essay should also address the fundamental difference between medical interventions and rehabilitative interventions. This section should also focus on advocacy for NOT simply relying on medical approaches. Treatment and rehabilitation should be complementary approaches.

Submission Artifact 10: Ethical Considerations – CLO 7

We have studied the ethics and guidelines associated with working with individuals with mental disorders, but when we are working with children there are other guidelines that come into play. To address ethical issues related to working with children utilize the Code of Ethical Principles of the American Academy of Child and Adolescent Psychiatry.

In this section write a brief summary of each of the 10 guidelines in a way that would communicate these concerns to the individuals working with this child in the school system. This should be formatted as a handout with attractive graphical elements and written in a way that is pragmatic for all levels of understanding.

You may submit this document as a page in your paper or include it as an attachment along with your assignment.
