Exploring interpretations of blockchain's value in healthcare: a multi-stakeholder approach Purpose Drawing broadly from the technology frame (Davidson, 2002) and organizing vision perspectives (Swanson and Ramiller, 1997) which consider the business value of information technology as resulting from actors' efforts to make sense of new technology, the study applies Ojala's (2016) business model framework to examine how different sets of actors understand the value of blockchain within the healthcare sector. Design/methodology/approach To include the perspective of different sets of actors, the research combines a systematic literature review to capture academic research, semi-structured interviews with blockchain experts, with an analysis of blockchain healthcare vendors. Findings The study finds a high degree of congruence between the perspective of different actors, with key sources of blockchain value concentrated around value proposition, particularly enhancing privacy and security; value capture, specifically cost savings, and value network, mostly enhancing data accessibility and reducing intermediation. Value delivery is the least emphasized value creation mechanism and concerns primarily improvements in supply chain transparency. Minor variations between actors' interpretations of value exist, mostly around the contribution of blockchain to support the value proposition and include the provision of social value, the creation of trust, supporting automation and improving employment. Originality/value Recognizing that the value of new technology is as much the result of actors' interpretations, as the objective outcome of its deployment, this study takes a multi-stakeholder perspective to examine blockchain's business value and highlights new aspects of value associated with blockchain deployments. Industry 4.0 Disruption and Its Neologisms in Major Industrial Sectors: A State of the Art Very well into the dawn of the fourth industrial revolution (industry 4.0), humankind can hardly distinguish between what is artificial and what is natural (e.g., man-made virus and natural virus). Thus, the level of discombobulation among people, companies, or countries is indeed unprecedented. The fact that industry 4.0 is explosively disrupting or retrofitting each and every industrial sector makes industry 4.0 the famous buzzword amongst researchers today. However, the insight of industry 4.0 disruption into the industrial sectors remains ill-defined in both academic and nonacademic literature. The present study aimed at identifying industry 4.0 neologisms, understanding the industry 4.0 disruption and illustrating the disruptive technology convergence in the major industrial sectors. A total of 99 neologisms of industry 4.0 were identified. Industry 4.0 disruption in the education industry (education 4.0), energy industry (energy 4.0), agriculture industry (agriculture 4.0), healthcare industry (healthcare 4.0), and logistics industry (logistics 4.0) was described. The convergence of 12 disruptive technologies including 3D printing, artificial intelligence, augmented reality, big data, blockchain, cloud computing, drones, Internet of Things, nanotechnology, robotics, simulation, and synthetic biology in agriculture, healthcare, and logistics industries was illustrated. A Blockchain and Machine Learning-Based Drug Supply Chain Management and Recommendation System for Smart Pharmaceutical Industry From the last decade, pharmaceutical companies are facing difficulties in tracking their products during the supply chain process, allowing the counterfeiters to add their fake medicines into the market. Counterfeit drugs are analyzed as a very big challenge for the pharmaceutical industry worldwide. As indicated by the statistics, yearly business loss of around $200 billion is reported by US pharmaceutical companies due to these counterfeit drugs. These drugs may not help the patients to recover the disease but have many other dangerous side effects. According to the World Health Organization (WHO) survey report, in under-developed countries every 10th drug use by the consumers is counterfeit and has low quality. Hence, a system that can trace and track drug delivery at every phase is needed to solve the counterfeiting problem. The blockchain has the full potential to handle and track the supply chain process very efficiently. In this paper, we have proposed and implemented a novel blockchain and machine learning-based drug supply chain management and recommendation system (DSCMR). Our proposed system consists of two main modules: blockchain-based drug supply chain management and machine learning-based drug recommendation system for consumers. In the first module, the drug supply chain management system is deployed using Hyperledger fabrics which is capable of continuously monitor and track the drug delivery process in the smart pharmaceutical industry. On the other hand, the N-gram, LightGBM models are used in the machine learning module to recommend the top-rated or best medicines to the customers of the pharmaceutical industry. These models have trained on well known publicly available drug reviews dataset provided by the UCI: an open-source machine learning repository. Moreover, the machine learning module is integrated with this blockchain system with the help of the REST API. Towards a Remote Monitoring of Patient Vital Signs Based on IoT-Based Blockchain Integrity Management Platforms in Smart Hospitals Over the past several years, many healthcare applications have been developed to enhance the healthcare industry. Recent advancements in information technology and blockchain technology have revolutionized electronic healthcare research and industry. The innovation of miniaturized healthcare sensors for monitoring patient vital signs has improved and secured the human healthcare system. The increase in portable health devices has enhanced the quality of health-monitoring status both at an activity/fitness level for self-health tracking and at a medical level, providing more data to clinicians with potential for earlier diagnosis and guidance of treatment. When sharing personal medical information, data security and comfort are essential requirements for interaction with and collection of electronic medical records. However, it is hard for current systems to meet these requirements because they have inconsistent security policies and access control structures. The new solutions should be directed towards improving data access, and should be managed by the government in terms of privacy and security requirements to ensure the reliability of data for medical purposes. Blockchain paves the way for a revolution in the traditional pharmaceutical industry and benefits from unique features such as privacy and transparency of data. In this paper, we propose a novel platform for monitoring patient vital signs using smart contracts based on blockchain. The proposed system is designed and developed using hyperledger fabric, which is an enterprise-distributed ledger framework for developing blockchain-based applications. This approach provides several benefits to the patients, such as an extensive, immutable history log, and global access to medical information from anywhere at any time. The Libelium e-Health toolkit is used to acquire physiological data. The performance of the designed and developed system is evaluated in terms of transaction per second, transaction latency, and resource utilization using a standard benchmark tool known as Hyperledger Caliper. A Systematic Review for Enabling of Develop a Blockchain Technology in Healthcare Application: Taxonomy, Substantially Analysis, Motivations, Challenges, Recommendations and Future Direction Blockchain in healthcare applications requires robust security and privacy mechanism for high-level authentication, interoperability and medical records sharing to comply with the strict legal requirements of the Health Insurance Portability and Accountability Act of 1996. Blockchain technology in the healthcare industry has received considerable research attention in recent years. This study conducts a review to substantially analyse and map the research landscape of current technologies, mainly the use of blockchain in healthcare applications, into a coherent taxonomy. The present study systematically searches all relevant research articles on blockchain in healthcare applications in three accessible databases, namely, ScienceDirect, IEEE and Web of Science, by using the defined keywords ‘blockchain’, ‘healthcare’ and ‘electronic health records’ and their variations. The final set of collected articles related to the use of blockchain in healthcare application is divided into three categories. The first category includes articles (i.e. 43/58 scientific articles) that attempted to develop and design healthcare applications integrating blockchain, particularly those on new architecture, system designs, framework, scheme, model, platform, approach, protocol and algorithm. The second category includes studies (i.e., 6/58 scientific articles) that attempted to evaluate and analyse the adoption of blockchain in the healthcare system. Finally, the third category comprises review and survey articles (i.e., 6/58 scientific articles) related to the integration of blockchain into healthcare applications. The final articles for review are discussed on the basis of five aspects: (1) year of publication, (2) nationality of authors, (3) publishing house or journal, (4) purpose of using blockchain in health applications and the corresponding contributions and (5) problem types and proposed solutions. Additionally, this study provides identified motivations, open challenges and recommendations on the use of blockchain in healthcare applications. The current research contributes to the literature by providing a detailed review of feasible alternatives and identifying the research gaps. Blockchain Technologies: Opportunities for Solving Real-World Problems in Healthcare and Biomedical Sciences Introduction: Blockchain technology is associated with the financial industry, but it can be applied to other industries. The supporting architecture of blockchain has the immense potential to transform the delivery of healthcare, medical, clinical, and life sciences, due to the extended functionality and distinct features of its distributed ledger. The potential scale of impact is comparable to that seen with the introduction of TCP/IP. Blockchain technology has captured the interest of healthcare providers and biomedical scientists within various healthcare domains such as longitudinal healthcare records, automated claims, drug development, interoperability in population health, consumer health, patient portals, medical research, data security, and reducing costs with supply chain management. It is not yet clear if blockchain is going to disrupt healthcare, but healthcare organizations are monitoring its potential closely for prospective concepts like secure patient IDs. Realistically, the adoption and implementation of blockchains will be a gradual evolution over time, but now is the time to take a fresh look at its possibilities in healthcare and biomedical sciences. Blockchain technology revolutionary solutions are bringing us closer to the possibility of every patient record being able to send updates to an open-source, community-wide trusted ledger that is accessible and understood across organizations with guaranteed integrity. Blockchain and supply chain management integration: a systematic review of the literature Purpose This paper aims to identify, analyse and organise the literature about blockchains in supply chain management (SCM) context (blockchain–SCM integration) and proposes an agenda for future research. This study aims to shed light on what the main current blockchain applications in SCM are, what the main disruptions and challenges are in SCM because of blockchain adoption and what the future of blockchains holds in SCM. Design/methodology/approach This study followed the systematic review approach to analyse and synthesise the extant literature on blockchain–SCM integration. The review analysed 27 papers between 2008 and 2018 in peer-reviewed journals. Findings Blockchain–SCM integration is still in its infancy. Scholars and practitioners are not fully aware of the potential of blockchain technology to disrupt traditional business models. However, the electric power industry seems to have a relatively mature understanding of blockchain–SCM integration, demonstrated by the use of smart contracts. Additionally, the disintermediation provided by blockchain applications has the potential to disrupt traditional industries (e.g. health care, transportation and retail). Research limitations/implications The limitations of this study are represented mainly by the scarcity of studies on blockchain–SCM integration in leading journals and databases. Practical implications This study highlights examples of blockchain–SCM integration, emphasising the need to rethink business models to incorporate blockchain technology. ‘Fit-for-purpose?’ – challenges and opportunities for applications of blockchain technology in the future of healthcare Blockchain is a shared distributed digital ledger technology that can better facilitate data management, provenance and security, and has the potential to transform healthcare. Importantly, blockchain represents a data architecture, whose application goes far beyond Bitcoin – the cryptocurrency that relies on blockchain and has popularized the technology. Blockchain-Based Healthcare: Three Successful Proof-of-Concept Pilots Worth Considering This paper features the use of blockchain technology in the healthcare industry, with special focus on healthcare data exchange and interoperability; drug supply chain integrity and remote auditing; and clinical trials and population health research. This study uses the research method of analyzing the published case studies, academic articles, trade articles, and videos on MEDRec, Patientory, and the AmerisourceBergen/Merck alliance with SAP/CryptoWerk. The "blockchain" concept was introduced around October 2008 when a proposal for the virtual currency, bitcoin, was offered. Hitching Healthcare to the Chain: An Introduction to Blockchain Technology in the Healthcare Sector Health services must balance patient care with information privacy, access, and completeness. The massive scale of the healthcare industry also amplifies the importance of cost control. Assessment of the Blockchain Technology Adoption for the Management of the Electronic Health Record Systems Blockchain is an emerging technology that holds great promises in healthcare despite slow adoption and previous unsuccessful implementation projects. Blockchain adoption in healthcare has been slow, partly because healthcare is a heavily regulated and complex industry. Blockchain applications span various areas of healthcare such as patient data management, health information exchange, health supply chain management, financial and insurance claims, clinical trial, biomedical devices tracking, and pharmaceutical counterfeit. The main challenges with blockchain technology in healthcare are: scarcity of real applications, the high level of failing projects, and the need for various parties to function together. There is, however, a lack of research on how to assess the adoption and help healthcare organizations use blockchain for the management of the electronic healthcare records (EHR) systems in a comprehensive way incorporating multiple perspectives. The objective of this research is to develop a scoring model to evaluate healthcare organization’s readiness to adopt blockchain for the management of the EHR systems. In this research, a literature review and expert feedback were used to identify the most important factors influencing blockchain adoption. The focus is on the application of the blockchain technology adoption for the management of the EHR systems. The Hierarchical Decision Modeling (HDM) methodology was used to elicit multiple expert’s judgment to identify the relative importance of those factors influencing blockchain adoption. In addition, experts’ feedback was used to identify the possible statuses an organization might have regarding each factor and the dynamic aspects of these factors was analyzed. Finally, two case studies of the blockchain adoption, Oregon Health and Science University (OHSU) hospital and a Medical City in Saudi Arabia, were conducted to demonstrate the practicality and value the research model brings to the research objective. The outcomes of the research present an identification of blockchain adoption impacting factors and their resultant rankings. The research identifies 17 factors as the most important factors influencing blockchain adoption and a healthcare organization’s readiness for adoption. The factors are grouped into five perspective: financial, social, technical, organizational, and regulations & legal. Exploring interpretations of blockchain's value in healthcare: a multi-stakeholder approach Purpose Drawing broadly from the technology frame (Davidson, 2002) and organizing vision perspectives (Swanson and Ramiller, 1997) which consider the business value of information technology as resulting from actors' efforts to make sense of new technology, the study applies Ojala's (2016) business model framework to examine how different sets of actors understand the value of blockchain within the healthcare sector. Design/methodology/approach To include the perspective of different sets of actors, the research combines a systematic literature review to capture academic research, semi-structured interviews with blockchain experts, with an analysis of blockchain healthcare vendors. Findings The study finds a high degree of congruence between the perspective of different actors, with key sources of blockchain value concentrated around value proposition, particularly enhancing privacy and security; value capture, specifically cost savings, and value network, mostly enhancing data accessibility and reducing intermediation. Value delivery is the least emphasized value creation mechanism and concerns primarily improvements in supply chain transparency. Minor variations between actors' interpretations of value exist, mostly around the contribution of blockchain to support the value proposition and include the provision of social value, the creation of trust, supporting automation and improving employment. Originality/value Recognizing that the value of new technology is as much the result of actors' interpretations, as the objective outcome of its deployment, this study takes a multi-stakeholder perspective to examine blockchain's business value and highlights new aspects of value associated with blockchain deployments. In particular, there is ambiguity among stakeholders regarding the responsibility to address this issue. The rationale for this grounded theory study was to investigate the soft skills gap by hearing employers’ perspectives on how to manage the phenomenon. Interviews from 27 STEM employers local to the state of Kentucky highlighted the prominent importance of soft skills in the future of work. The demand will continue to increase as employers place higher premiums on new hires with soft skills. The five most valued soft skills of 2020 - 2025 that employers identified are leadership, human-connection, communication, creativity, and collaboration. Employers placed special emphasis on the graduates' ability to draw upon their humanity in conducting daily work tasks. To instill these capacities and address this phenomenon, the results from these interviews suggest that there is a need to generate a systematic synergy between employers and educators. A strategy to do so could focus on partnerships rooted in coordinated, shared-value communication. Employers who have already pursued this approach have noted benefits like optimum ROIs, secure employment, client satisfaction, employee well-being, and more. Business Process Management and Digital Innovations: A Systematic Literature Review Emerging technologies have capabilities to reshape business process management (BPM) from its traditional version to a more explorative variant. However, to exploit the full benefits of new IT, it is essential to reveal BPM’s research potential and to detect recent trends in practice. Therefore, this work presents a systematic literature review (SLR) with 231 recent academic articles (from 2014 until May 2019) that integrate BPM with digital innovations (DI). We position those articles against seven future BPM-DI trends that were inductively derived from an expert panel. By complementing the expected trends in practice with a state-of-the-art literature review, we are able to derive covered and uncovered themes in order to help bridge a rigor-relevance gap. The major technological impacts within the BPM field seem to focus on value creation, customer engagement and managing human-centric and knowledge-intensive business processes. Adoption of the Health Information Exchange (HIE) System and the Role of the Healthcare Leadership The advancement in technology calls for improvement in service delivery in healthcare systems, especially in the present health pandemic of the COVID-19. At present, healthcare systems utilize several electronic health techniques. However, there is a need for the creation of interoperability that can assist healthcare organizations in exchanging data more effectively through the global adoption of healthcare information exchange (HIE). The current research examines the adoption of HIEs and healthcare leadership roles to realize a fully functional interoperable patient HIE. This study utilized the systematic document analysis method to review qualitative data on the different aspects of healthcare leadership, implementation of HIEs, professional development, and the benefits of Healthcare Information Exchange to the organizations that use HIEs. The study analyzed data using the NVIVO-12 software and identified four major themes, namely: HIE Project Implementation, Healthcare Management, and Professional Development, HIE Systems are Beneficial for Operations with Technology being the Critical Factor , representing all four research questions respectively. The analysis of these themes revealed that leadership plays a crucial role in the implementation of HIEs. Federal and state policies are significant elements that affect HIE implementation at the organizational level. The analysis revealed that healthcare professionals need opportunities to enhance their knowledge in core areas to their healthcare organization's success. Additionally, HIEs are beneficial across all levels, including national, organizational, and patient levels. The current research recommends a focus on leadership skills, enhanced careers, and collaboration between professions to improve the implementation of effective HIEs used across all healthcare organizations. Blockchain and Learning Organizations: How the Emerging Technology Impacts Knowledge Sharing As technology continues to evolve, so do learning organizations that use technology to enhance their knowledge management strategies, particularly regarding knowledge sharing using computer-mediated communications. This constant disruption requires learning organizations to be uniquely adaptable. Yet one application is receiving little attention in the research literature: blockchain. Best known as the foundation of the cryptocurrency Bitcoin, blockchain offers a uniquely technological, programmable, and interactive way to create distributed and reliable applications. Since Satoshi Nakamoto’s 2008 introduction of a peer-to-peer electronic cash system, there has been a surge of research into blockchain’s dynamic potential beyond an immutable ledger of cryptocurrency transactions. However, few research studies to date have focused on the intersectionality of knowledge sharing and blockchain technology, including those elements that could shape how learning organizations share knowledge. Using a realist synthesis approach, a conceptual model is posed that highlights blockchain’s impact on knowledge sharing in learning organizations. Comparative Study of Healthcare Messaging Standards for Interoperability in eHealth Systems Advances in the information and communication technology have created the field of "health informatics," which amalgamates healthcare, information technology and business. The use of information systems in healthcare organisations dates back to 1960s, however the use of technology for healthcare records, referred to as Electronic Medical Records (EMR), management has surged since 1990’s (Net-Health, 2017) due to advancements the internet and web technologies. Electronic Medical Records (EMR) and sometimes referred to as Personal Health Record (PHR) contains the patient’s medical history, allergy information, immunisation status, medication, radiology images and other medically related billing information that is relevant. There are a number of benefits for healthcare industry when sharing these data recorded in EMR and PHR systems between medical institutions (AbuKhousa et al., 2012). These benefits include convenience for patients and clinicians, cost-effective healthcare solutions, high quality of care, resolving the resource shortage and collecting a large volume of data for research and educational needs. My Health Record (MyHR) is a major project funded by the Australian government, which aims to have all data relating to health of the Australian population stored in digital format, allowing clinicians to have access to patient data at the point of care. Prior to 2015, MyHR was known as Personally Controlled Electronic Health Record (PCEHR). Though the Australian government took consistent initiatives there is a significant delay (Pearce and Haikerwal, 2010) in implementing eHealth projects and related services. While this delay is caused by many factors, interoperability is identified as the main problem (Benson and Grieve, 2016c) which is resisting this project delivery. To discover the current interoperability challenges in the Australian healthcare industry, this comparative study is conducted on Health Level 7 (HL7) messaging models such as HL7 V2, V3 and FHIR (Fast Healthcare Interoperability Resources). In this study, interoperability, security and privacy are main elements compared. In addition, a case study conducted in the NSW Hospitals to understand the popularity in usage of health messaging standards was utilised to understand the extent of use of messaging standards in healthcare sector. Predominantly, the project used the comparative study method on different HL7 (Health Level Seven) messages and derived the right messaging standard which is suitable to cover the interoperability, security and privacy requirements of electronic health record. The issues related to practical implementations, change over and training requirements for healthcare professionals are also discussed. The study finds that HL7’s FHIR (Fast Healthcare Interoperability Standards) is the most suitable messaging standard satisfying all the modern demands for healthcare interoperability requirements. These interoperability requirements are communication methods, security, usage of encryption, privacy, compatibility, flexibility and reliability. FHIR has satisfied all these requirements well and it is the most preferred one among other HL7 standards. The adoption rate of FHIR is expected to be high as it has the flexible transport and client-side features. However, FHIR is still in draft status and it may take considerable time to plan for resource repository and training to healthcare professionals.
