The formulation of theories can achieve understanding, explaining, and predicting given phenomena. Also, theories are used to extend and challenge existing knowledge, within limits of essential bounding assumptions. The theoretical framework is a structure that plays a critical role in holding or supporting research study theory. It describes theories that explain the reasons for the existence of any research problem under study. A theoretical framework is made up of concepts and theories. The framework must demonstrate concepts and theories understanding that are essential to the topic of the research.

In this research proposal, I am using transition theory to review various articles on readmission and heart failure. Using this theory can assist patients, clinicians, and families in adjusting to the challenges faced in heart failure diagnosis (Fitzpatrick & McCarthy, 2014). I have conducted extensive research with the help of the theory to investigate the causes of heart failure and the readmission of patients. Other researchers in the reviewed articles have provided various solutions.

Application of theory to study focus

In my study, I focus on heart failure diagnosis and readmission of patients. I focused on how the condition of heart failure among patients can be mitigated. Some of the best ways to the solution is reducing readmission cases and fees to ill-health patients (Arora et al., 2017). Using transition theory can assist me with a useful context to adjust to challenges inherent in heart failure diagnosis and illness trajectory negotiation.

The transition theory ensures acknowledging various factors such as dealing with vulnerability, resolving uncertainty, and restructuring reality. Thus, incorporating the factors in intervention, information, and care planning can facilitate effective treatment and diagnoses. Also, reducing readmission cases can be effectively solved.
