Presenting issues and purpose of contact:

This is our first meeting she lives in Snohomish she is 45 years old but can not work because she is in the advanced stages of muscular sclerosis. She needs help applying for Medicaid and SSDI, she needs resources to get help in her home with housekeeping. Her case manager would like us to assess the home too because the client has had several falls.

I am trying to assess how often she falls.

D: I’ve been falling a lot lately?

A: I am very sorry to hear that, could you kindly tell me about the times you fell?

D: I try to save the electricity bills as much as possible, so I keep it turned off. I clean up my room once or twice a week since I experience extreme fatigue and pain in my muscles.

A: I am very sorry to hear that. Have you sought medical assistance for your extreme fatigue and pain in your muscles?

D: I cannot afford to pay for treatment. I lost my job. I have been unable to go back to my previous job and finding a suitable job has been difficult. Moreover, I don't know when my condition will be healed.

D: No, it has been difficult because I don't know-how.

A: Don’t worry, everything will be alright. We will help you in applying for them. Also, would you try opening the curtains in the morning and arranging your stuff around your room and see if it helps with your falls. We’ll come back next week on Tuesday to check on the progress.

Student Self-Assessment:

I started to find out the cause of her several falls. Therefore, I assessed her home living conditions. I found out that she mostly falls after taking the shower and while getting out of her bed. Thus, I thought the issue could have been a result of poor lighting and lack of grabs in the shower. Additionally, she was disorganized, which could have contributed to her falls. She complains that she cannot tidy up her room a lot because she has muscle pain and fatigue. Therefore, I was concerned about her course of treatment. I assessed her application for Medicaid and SSDI. She has a low income and she is always worried about her bills. Additionally, she has been unable to apply for Medicaid and SSDI. I hope she receives more guidance on how she can go about applying for Medicaid and SSDI since she has a low income.
