Problem statement

Briefly describe the context in which the problem is occurring. When did the organization become aware of the problem? What actions, if any, have been taken to mitigate the problem?
Process water valves having to be operated manually causing delays when the plant process has upsets or changes in the amount of silica being produced. This process of having to adjust the valves manually causes delays in making appropriate and timely changes required to efficiently control the amount of water needed in all areas of the wet plant production process.
Step 4: Identify the person in the organization, by position or job title only, that authorized you to resolve the problem:

Step 5: List at least three areas of leadership responsibility that are contributing to or affected by the problem. Leadership areas may both contribute to and be affected by the problem. At least one leadership areas has to be contributing to the problem.

How the leadership areas of responsibility are contributing to the problem or affected by the problem must be clearly analyzed and supported in the consulting report with evidence and data. Not, needs to, must, and lack type statements are not evidence. Such statements only point toward solutions but fail to explain how the leadership is currently contributing to or affected by the problem.
