A.SOCIAL DEMOGRAPHIC PROFILE
1. Key Findings
· The percentage of Adult smoking has increases from 54% to 63% in the past 12 months
· 84% of people of 65 years + have diabetes and high blood pressure.
· 81% of person living with disability of age 65 years + have no health insurance
· The number of children with obesity between age (8-14) has increased from 42% to 47 % in the past 12 months.
· Sexually transmitted infections between persons of age (16-25) has increased 36% from 11.5to 14.0 per 1000.
2. Introductory Paragraph
The source of data is based on a ranking model of population health. From the finding, injury death in the county is very high as compared to the number in the whole country. The number of people living with disability 65 years and above is also that of the United States. There are many people living with disabilities in the county.

B.COUNTY HEALTH TRENDS AND RANKINGS
1. Introductory Paragraph
He data was collected to find out and compare how heath trends in the county can be compared to the United States. The data was also collected to find trends of the health trends and the effects on the total population. These rankings were compiled using county level measurement and scientifically informed weights.
2. Discussion of Health Trends
The percentage of poor mental health in the county is high as compared to the U.S. The trend in improving since the percentage is dropping from 22% to 18%.The trend in adult obesity in the county is worsening as the percentage has increased from 30% to 32%.Sexually transmitted infections in the county is worsening, the number has risen from 256.6 to 534.6. Alcoholimpared driving death trend is improving and the percentage is decreasing

D.SUMMARY
1. Significant Findings
The located parmens use the findings to compare the different health trends and progress and come up with techniques to address the challenges. Alcohol impaired driving deaths can be reduced by campaigning against excessive drinking and prohibit drinking and driving. The alcohol addicts can be rehabilitated.
2. Action Plan(s)
The action plan is to reduce alcohol impaired driving deaths. This will be done by the state prohibiting drinking and driving and excessive drinking

Rehabilitation program and campaign against excessive drinking.
B. Raise Public Awareness
Professionals can launch online campaigns to educate the public of the health trends .The campaigns should be valuable to the affected and encouraging for the people to join. They should engage the target audience that is affected and find effective ways to solve their problems. They should also look for interested volunteers from the target audience to influence the people to join the movement. Joshi, Thorpe, L., & Waldron, L. 2017).
C. Monitoring and Evaluating Action Plan
The action will be monitored by frequently visiting the rehabilitation centers and checking the progress of the patients.
