The questionnaire is a structured instrument that is used to gather information about
respondents through a set of pre-established questions (Aung et al., 2021). It can show both
quantitative and qualitative date depending of the type of questions. This document employed the
Pre-vaccination Checklist of COVID019 Vaccines as a data collection tool to determine the
possible health determinants that may affect the vaccination readiness. The checklist rests on the
centers for Disease Control and Prevention( CDC, 2021) guidelines and aids in assessing
whether the individuals have any medical history or past experiences that can make them
cautious before taking the COVID-19 vaccine.
Four key questions were analyzed from the checklist:
1. “Are you feeling sick today?”
2. “Have you ever before received a dose of the COVID-19 vaccine?”
3. “Have you ever had an allergic reaction?”
4. “Have you ever had an allergic reaction to another vaccine?”
Six respondents were interviewed to give a total of 24 responses. Among them, 5 replies
(20.8%) were yes, and 19 replies ( 79.2%) were no. A bar graph was chosen as the best way of
getting this information because it would be effective in presenting the information. Bar graphs
are best used in-displaying categorical data, as one can easily compare the frequencies or the
percentages of various categories through the bar graph.
The findings are illustrated in the figure below, where the number of No answers is
evidently higher than that of Yes answers. It shows that the vast majority of the participants did
not indicate that they have any health conditions or concerns that would prevent them from
taking the COVID-19 vaccine.

All in all, the results indicate that most of the evaluated patients were an appropriate age
to receive vaccination, and no major contraindications have been indicated. The bar graph is easy
to understand and use as a means to summarize and convey these findings, and is useful in
primary care (Scalia et al., 2021).
