Telenursing and Transitional Nursing Care

Introduction

Telenursing and transitional nursing care are essential interventions for efficient healthcare

delivery in a healthcare organization. Telenursing is typically the use of technology in the

provision of the nursing services, especially when there is a physical distance between the nurses

and the patients. Telenursing helps in driving down healthcare costs and increase the number of

chronically ill and the aging population. Moreover, it enhances the increase of coverage of

healthcare to distant, sparsely populated areas. It also helps in solving the issues of increasing

nursing shortage, hence reduces the distances to save the travel time and keep the patients out of

the hospital.

Transitional nursing helps to prevent complications, including re-hospitalizations of the

chronically ill, including hospital patients through the provision of comprehensive discharge

planning and even home follow up. Therefore, the transitional nursing care model focuses on the

provision of quality care by well-trained nurses. For example, during a hospital stay, the nurses

conduct a comprehensive assessment of the patient’s health status, level of social support, and

health behaviors (Naylor, & Van Cleave, 2010). The nurses also develop an individualized plan of

care that is typically consistent with the evidence-based guidelines. They also conduct daily patient

visits, which are typically focused on optimizing the health of the patients at discharge.

Therefore, implementing telenursing and transitional nurses will help improve the quality

of health care in health institutions. There are important steps, which will be involved in the

implementation process (A guide to the implementation process: stages, steps, and activities, n.d.).

The project steps will be such as setting or revising the goals, identifying risks, selecting software

development projects, decide when to launch the process and the tools, plan for training, plan for

mentoring and then execute the process implementation and lastly, evaluate the implementation

process effort.

Setting or revising the goals

The first step of implementing telenursing and transitional nursing will be setting or

revision the made goals. The goals will be important in providing a long-term vision, including

short-term motivation (A guide to the implementation process: stages, steps, and activities, n.d.).

Moreover, it will enhance the organization of the time of the project’s activities. The goals of

implementation include the installation of efficient information technology and

telecommunication systems to provide nursing care. Secondly, is to solve the issue of nurses’

shortages. Thirdly is to reduce travel distance and time. The forth is to keep the patients out of the

hospital. Other goals are such as effective interactions with the patients, including their families,

and a successful transition engagement (Making care transitions safer: The pivotal role of nurses,

n.d.). There is also avoidance of preventable poor outcomes, among all the populations at risk, and

promotion of a safe and timely transfer of patients, particularly from the lone level of care to

another or from one type of setting to another. In general, telenursing and transitional care will

improve the health of populations such as the children, older people, people with multiple health

conditions, and those living in nursing homes or residential care.

Identifying risks

The second step in the implementation process will be the identification and mitigation of

risks. For example, the risks, which will be associated with the installation of information

technology and telecommunication systems, will be identified (Elzamly, A., & Hussin, B. (2013).

There are risks, which may involve the pilot project, which may involve many technical risks,

many things, which people will be supposed to learn, the uncertainty of whether the tools will

work well together, and doubts of how to use the tools (A guide to the implementation process:

stages, steps, and activities, n.d.). Therefore, such risks will be identified and then analyzed to

know on their seriousness. The risks will then be prioritized so that the most serious risks will first

be attended to, followed by the less harmful risks. It will be important to involve risk avoidance

strategies to help mitigate the risks hence prevent harm to the implementation process of the project

to ensure its success. Contingency strategies will also be beneficial to enhance the efficient

management of risks.

Selecting software development projects

The next will be selecting a software development project, which will involve engagement

in pilot projects, to help the project managers and researchers to try out new ideas, through new

tools and processes. Consequently, the best processes and tools will be used (8. Planning and

implementation, n.d.). Additional resources must be available to help adjust the budget

accordingly. Hence different information technology and telecommunication system will be

involved to enhance the identification of the best tool to be used. For example, new pilot projects

will be undertaken using approaches such as typical, fast, distributed, and careful approaches.

Besides, the most appropriate ones will be used.

Decide when to launch the process and the tools

The fourth step will be deciding on when to launch the telenursing operating systems,

including transitional nursing interventions. For example, the project managers will decide on how

and when they will launch the telenursing tools, including transitional model intervention. To

ensure the efficient launch of the development project, such as the pilot project, it is essential to

engage the team members in a training activity in preparation for the actual project. The

development process implementation guidelines will also be essential, as they will provide

guidelines to the project’s team members, who will be responsible for implementing both the tools

and the process (8. Planning and implementation, n.d.). Mentors will also be prepared so that they

will provide support to the project managers and participants to ensure efficient and successful

project implementation.

Plan for training

The fifth step is planning for training for the actual project implementation. Therefore, the

competency levels of the team members will be evaluated. Next, the missing skills among the

members will be identified and included in the training. Hence, it will be decided which training

is needed depending on the available telenursing tools and procedures available. It is important to

note that a change process and tools typically affect the entire operations of the project

implementation process (8. Planning and implementation, n.d.). Therefore, the training activities

will be helpful to provide an understanding of the changes, which will be involved, including

necessary activities to be undertaken. For example, the training may include an overview course,

which will be combined with seminars or even workshops to introduce the new telenursing and

transitional nursing tools and processes. Besides, the training will allow the team members to

acquire new forms of skills, including sharpening the existing ones, so that they perform better, to

help increase the levels of productivity. Hence, the project managers will ensure that the team’s

members and the participants within the project perform at their peak to ensure the success of the

project.

Plan for mentoring

Planning for mentoring will be the sixth step. It will involve the involvement of more

experienced individuals to me, mentors, to the project team members. Hence, it is important that

in software development activity, there is a process mentor, so that he or she helps the team

members to initiate and end the implementation procedures well (8. Planning and implementation,

n.d.). For example, the information and telecommunication systems for telenursing maybe

complex, hence setting them up will be a challenge. Therefore, allocating resources for tool

mentoring and support will be beneficial and helpful.

Moreover, mentoring will provide professional socialization, including personal support,

which will then enhance the facilitation of success in graduate school and beyond. It is important

to note that quality mentoring typically enhances individuals’’ chances to be successful in every

activity they engage in. Besides, those mentored will be encouraged and gain practical advice and

learn from the experience of the mentors. Hence they will develop effective skills for dealing with

various challenges that may occur during the project process implementation.

Execute the process of implementation

The seventh step is executing the project. Hence the telenursing tools will be assembled

and within a health facility (8. Planning and implementation, n.d.). The transitional nursing care

model will be engaged among the patients. The transitional activities will include nursing roles

such as promoting successful transitions through developing and even evaluating the transition

plan. Secondly, the nurses will engage the patients, including the caregivers, as the active partners

for the community support and including healthcare needs. Moreover, the nurse will also focus on

longer-term outcomes, through identification and documentation of the transition issues. They will

also communicate the plan for the transition to the next level of care.

Evaluate the implementation process effort

The final step of the implementation process will be the evaluation of the implementation

process. When the team members have completed conducting a real or pilot development project,

it will be important to evaluate the effort (8. Planning and implementation, n.d.). Hence, it will

help to show whether the established goals were achieved. Moreover, it will involve the evaluation

of the project participants, the process of the project, and the launched tools, so that it will provide

an understanding of the areas to be focused on during the next phase of the implementation effort.

Conclusion

Telenursing and transitional nursing care are quite beneficial in healthcare delivery.

Besides, it ensures the provision of quality care and improvement of health among the patients.

Hence, implementation of telenursing and transitional nursing will need a good team, which will

ensure the success of the project (Naylor, & Van Cleave, 2010). Hence, the project steps such as

setting or revising the goals, identifying risks, selecting software development projects, decide

when to launch the process, and the tools. Others are planning for training, planning for mentoring,

and then execute the process implementation, and lastly, evaluating the implementation process

effort, will all ensure the success of the project.
