The 2018 estimates show that Minnesota had a total population of 5.6m people with over 2 million households. From 2010, the population has been growing at the rate of 6.1%("Minnesota County public health profiles - MN data,“ n.d.)

Most of Minnesotans work in the health sector (15%) with minority in the local government.

Economy

In 2018, about 4.7% of Minnesotans were uninsured. This accounts for more than 250,000 people. Ambulatory disability is the most common disability among 595,000 disabled people.

The poverty rate in Minnesota stands at 10% in 2018. The median income per household is at $70,300(Minnesota. State Demographic Center, 2018)

The population is estimated to be more than 6 million by 2032. This will cause the need for improved population health.

In 2010, less than 25% of the population was under the age of 18 years. Non Hispanic white people were 83% while the rest including American Indians were 16%

Health Status of Minnesotans

Infant mortality rate is 5% with over 6.5% of births being preterm

Breast cancer is the predominant at 132 per 100, 000 followed by lung cancer(MN State Demographic Center, 2011).

Only 69% of children receive full immunization as per the scheduled guidelines

At 3 years of age, more than 80% of children have been exposed to lead.

Infant mortality rate is higher compared to other states. Mother to child healthcare services should be improved to reduce the mortality rate.

The number of children with full immunization is also high. Sensitization of the public on the benefits of immunization will be crucial in increasing the number

More then 70% of Minnesotans over the age of 60 years have an underlying chronic condition

However, the prevalence among children below 18 years stands at 18%

About 35% of the population have at least one chronic condition, of this 42% have one chronic conditions while 37% have three or more

Females are more susceptible to the chronic conditions at 39% with males at 32%.

High blood pressure is the most prevalent condition followed by high cholesterol, asthma and then diabetes(Minnesota Department of Health, 2012).

The prevalence of chronic conditions among Minnesotans is considerably higher at 70% for people above 60 years.

Most of these conditions are as a result of lifestyle and socioeconomic patterns.

Population Health Programs should be aimed at reducing these levels.

All hospitals in Minnesota have implemented electronic health records since 2014. About 70% of hospitals use epic EHR..

Over 90% of the hospitals have implemented e-drug alerts which is crucial in promoting access to healthcare.

About 97% of the hospitals have patient portal. This ensures that they are able to obtain healthcare service as well as information without geographical barrier(Minnesota Department of Health, 2012).

More than 90% of laboratories in Minnesota have laboratory information systems.

However, the greatest challenge has been the services and workforce to assist in mapping local codes.

The use of EHR and LIS for the hospitals and laboratories have greatly improved clinical outcomes over the years.

Employment of more health care providers and IT specialists to map local codes will be necessary to further meet the healthcare needs of the population.

Intervention Programs

The major disease condition in Minnesota is high blood pressure. Dietary measures, physical exercise and pharmacological interventions can be utilized to manage the condition

Yang et al., (2017) highlight lifestyle behavior including sedentary lifestyle and diet are associated with high blood pressure.

Programs such as sensitizing the population on nutrition health will be necessary to reduce the risk of such disorders.

Diabetes as well as high cholesterol can all be managed by lifestyle changes without visiting the health facility. This will reduce healthcare costs while improving the health status of Minnesotans (Shi, 2016)

These are lifestyle disorders which can be easily managed by lifestyle changes.

Dietary changes as well as encouraging the population to engage in exercise will help reduce the incidence of diabetes and high blood pressure.

Sensitizing the public on the risks and complications of diabetes can be essential to limiting its rate within the population

Dietary interventions as well as physical exercise can be utilized during management and to shield high risk individuals

High blood pressure education program

Minnesotans are at high risk of developing high blood pressure as indicated by statistical data.

Educating the population on intervention and prevention strategies including pharmacological mechanisms can promote the population health

Diabetic and high blood pressure prevention programs will be helpful in boosting the status of health in Minnesota.

The programs should be prioritized for people above 60 years who have a higher risk of developing diabetes and other chronic conditions.

Mass media education programs on risks and complications of chronic conditions will be helpful in sensitizing the public.

Training of healthcare providers on management of chronic conditions will also be vital in ensuring better clinical outcome among patients with chronic illnesses.
