Request for the process for reinstatement to your CMHC program following academic disqualification. 

A program reinstatement petition will allow you to make a case for reinstatement. It is an opportunity to think critically and share how you will prioritize your academic program to ensure that you are successful.

Provide a statement explaining the circumstances under which they were dismissed (Class fail) and the reasons you should be supported for reinstatement.

Please spend some time drafting this statement. As I mentioned earlier, it should include a plan that outlines how you will ensure academic success if reinstated. The department will review your materials and if supported for reinstatement, write a letter which outlines the timeline for reinstatement and a remediation plan.
