Summary of the history of the client, with subjective and objective findings
Mr. G is among many other elderly adults who have visited the health facility before with the same complaints. This client visits the health facility with severe complaints of lower leg pain. Presently, Mr. G is fighting with the condition that does not allow ho working effectively in his garage. According to the subjective findings, the client feels pain in his joints. He also has pain in his legs after several activities. The client also talks about how uncomfortable while standing. His legs pain a lot while he is standing. When he moves, he also says that the pain is still past average. His condition becomes a bit normal when he is relaxed. This client is undergoing a situation that might seem healthy because of his age. However, subjective findings proved that wrong. The patient’s condition was observed. Some simple exercises were given to the client. According to the practice, it was noted that the patient was suffering from severe leg pain. According to the observation, it is proved that the client had severe bilateral lower pain (Belli & Adam, 2015). Subjective findings using x-ray gave various results for instance the tests also showed that the client had joint paints due to blocked veins. Urinalysis tests show that the patient had high blood pressure. It also proved a large number of lipids in the blood. The move is to ascertain if Mr. G is suffering from peripheral vascular system complications.
An assessment of the peripheral vascular system of the client
An assessment is required to ensure that the patient’s peripheral vascular system. In this assessment, I would focus on coming up with a plan to assess the condition. First, I would recommend a frequent checkup to determine the level of the pain he is undergoing. His peripheral vascular system condition will also be determined through laboratory tests. This move will ensure that the client's health will be determined. The second step would be assessing the situation of his system. These tests will assess the level of blood glucose, cholesterol, and tightness of the veins. This assessment will help determine if the side effects have any relation to the pains in his legs.
The tests to be taken include:
To determine that Mr. G is suffering from the disease, the following tests will be conducted:
i. Blood tests- blood samples will be taken and tested to determine the level of cholesterols in the body. This move will also be helpful to check for blood sugar
ii. Physical examination- this exercise will aid to determine some signs of PAD. These sings include weaknesses, wounds taking long to heal, bruits which are determined using a stethoscope and low blood pressure among others.
iii. ABI- this test helps compare the blood pressure in the arm with that in the ankle. The patient will undergo this test to determine the possibility of peripheral vascular system complications.
iv. Another test to be done is the Ultrasound imaging techniques. Through this test, arteries are examined. The blocked ones will be identified and the flow of blood is examined.
Interpretation of the data from the assessment
Various differential diagnoses need consideration. According to the data recorded, as per the condition of the patient, hypertension, high blood pressure, high cholesterol, and low oxygen concentration could be the leading cause of the symptoms faced by the client. These differential diagnoses are of the primary consideration in treating this client. These risks include joint disease, cancer, heart attack, and Hypertension (Buck, 2016). Some health behaviors which lacked in this client's life include lack of a nutritious diet, no enough exercise, and lack of enough rest and sleep. In my assessment, I identified that the patient was under some medication. He, however, did not take supplement foods, which would improve his health condition.
A care plan for a possible differential diagnosis for the patient
The client needs a lot of care, especially to control the level of lipids in his blood. One of the things that need to be considered includes coming up with a diet that will help in controlling his cholesterol level. This action will be taken in collaboration with the nutrition department. The patient will also be exposed to exercises that will help in burning the extra fats in his blood. This action will be with the help of the physiotherapy department. This approach will help the patient also to control his blood pressure. This care plan will assist in improving the condition of his peripheral vascular system.
Recommended health promotion tool
Various health promotional tools are useful in patient education. These tools have been tested and proved to be effective. For Mr. G, I would recommend a brochure as a health promotion tool. The first goal of this tool is to give information about the condition of the patient. This goal will ensure that the patient has a complete understanding of his illness. When the patient has an understanding of this condition, he will be careful about the health risk factors, which would make the situation worse. The second goal of this health education tool will be to give the patient education about the disease. This goal will ensure that patient has medical knowledge about his condition. He will be able to learn about self-care management. The patient will be well educated about other disposing of factors and behaviors which would promote his health and condition (Casserly, Sachar & Yadav, 2012).
