Reason for Staffing: Youth was placed on supervised probation on 2/9/2017 for Theft Misdemeanor. Youth had been referred to FFT while on supervision in March of 2017. The services were successfully terminated in September of 2017. Shortly after being discharged from FFT, the youth and mother started to have difficulties again. Youth has recently taken her mother's car again and their relationship is strained. In court, the mother asked the Courts of FFT could work with the family again, specifically Tammie Nowlin who had previously worked with the family. SECTION X: SUMMARY AND RECOMMENDATIONS:

In connection with Case N o .4 0 M £ IP it. the Respondent was found involved to Theft Misdemeanor. In addition to the standard conditions, he was ordered to the following special conditions: Obtain/maintain employment, obey curfew, submit to drug alcohol/mental health evaluation (and engage in any recommended treatment/education) (if desired by the Department), random urinalysis (upon the Department's request and a t youth's expenseO, Take all medications as prescribed, attend Shock Trauma Tour, Participate in individual/family counseling (FFT or IFCS), write letters of apology and prepare career research project.

The Department completed a Needs Assessment on February 22, 2017, and based upon the social history and the delinquent history of the Respondent she poses a low risk for reoffending. The Respondent scored in the high range for needs in the areas of aggression. The Department developed a Treatment Service Plan to address those needs. Along with assisting the Respondent with the completion of her conditions of probation, the Department will also help her eliminate the use of illegal substances and reduce the frequency of expressions of anger.

Domain History Health: Youth has vision problems. Unclear if youth wears glasses. No other significant physical health problems reported at this time. Youth has never been diagnosed with a mental health disorder. Youth is not currently prescribed medication. No history of pyshciatric hospitalizations.

Family/Guardian History: Youth resides with her mother. No one else resides in the home. Father is incarcerated. Mother and youth have a strained relationship. Youth has a history of running away from home. Youth had FFT services last year which were recently closed out in September. Youth and mother continue to struggle and youth was charged in December for taking her mother's vehicle. Mother requested to the Courts that FFT services be initiated again.

Education: Youth is currently enrolled a f ^ ^ ^ a k e High School. Youth is a 9th grade student and is not receiving special education services. Youth has a history of attendance issues, however attendance has improved since being placed on GPS by the Courts.

Mental Health: No current diagnosis reported. Youth has not had a mental health assessment completed. Youth is not taking medication at this time. Youth does not have a history of psychiatric hospitalizations.

Substance Abuse: Youth has a history of using substances. Youth is currently being tested by DJS. Youth admits to using marijuana. If youth continues to test positive for marijuana she will be referred to have a substance abuse assessment completed and follow recommendations.
