Parent Interview Questions

1. Age, race, sex, grade and diagnosed disability/medical need/educational need of child.

2. What is your relationship to the child?

3. At what age was the child when you noticed they had difficulty in school? If the child has a disability, what age were they diagnosed?

4. Were there any signs or characteristics that indicated the need to seek a diagnosis? Discuss. (If applicable or discuss if condition is due to an accident, late onset medical need etc).

5. What do you consider to be one of your greatest challenges in being a parent of a child with a disability or a child with educational difficulties?

6. What have been some of your treasured experiences that you have had with your child?

7. What do you consider your child’s greatest strength(s)?

8. What do you consider your child’s biggest challenges?

9. What types of services (if any) does your child receive in and outside of school? (examples: speech, occupational therapy). Do you think that your child has benefitted from the services received since being diagnosed? Why or why not? Discuss any observations made in regard to the progress or lack of progress?

10. What is one of the major goals you want your child to accomplish by the end of his/her school education?

11. What types of information and resources have you found most useful? Describe what has made these resources useful.
