Introduction

This is where you introduce what you are going to cover in this paper (1-2 short paragraphs). You should also include, again, what problem you are working on.

Performance: Propose an Initiative

Proposes an initiative to address the chosen problem, including the quality standard being addressed. This section is where you will describe the initiative you plan to implement that will solve the problem. For example, you are going to implement a new policy and procedure. You will need to provide details about what your plan looks like. You should not simply state, I’m going to implements new policies to address this problem

Performance: Discuss Data Determinants of Success

Accurately discusses the data determinants of success related to this initiative. This part is where you provide information about what data you plan to collect to determine if your initiative is successful. For example, if your problem is “Wrong Site Surgeries” you might say that your data determinants will be: 1) # of wrong site surgeries prior to implementation; 2) # of “near misses”; 3) # of wrong site surgeries 3 months after implementing the initiative; and 4) # of wrong site surgeries 6 months after implementing the initiative.

Implementation: Describe Interdepartmental Communication Channels

Thoroughly describes the interdepartmental communication channels to be used for plan implementation. In this section you will discuss how you plan to communicate implementation of your initiative to staff (e.g., full staff meetings, department meetings, email reminders, etc., etc.).

Implementation: Describe Manner of Data Interpretation

Accurately describes the manner of data interpretation that will be used to communicate findings within the organization. Here is where you will describe how you plan to interpret the data you collected under the “Data Determinants” section. For example, after data is gathered you might use pie charts, line or bar graphs, etc. to show if there was progress.

Implementation: Effects of Initiative on Patient Care Outcomes

Comprehensively describes the hypothetical effects of this initiative on patient care outcomes, including how health information systems support improvements in patient care. The discussion here should be about the impact your initiative had/has on patient care outcome(s) (e.g., were the patients better served, did they get better, did they avoid a wrong site surgery).

Implementation: Effect of Quality Initiatives on Culture of Safety

Comprehensively describes the hypothetical effect of the quality initiative on the culture of safety within the organization. This section should be about how your initiative impacted the culture of safety within the organization.

SPECIAL NOTE: Critical Element #7 (Articulation of Response) is (Proficient) is a “submission has no major errors related to citations, grammar, spelling, or syntax (he grammatical arrangement of words in sentences)”. (Exemplary) is a “submission is free of errors related to citations, grammar, spelling and syntax and is presented in a professional and easy-to-read format.” AGAIN DELETE THE HIGHLIGHTED AREA AND THE DESCRIPTIONS ABOVE PRIOR TO SUBMISSION.
