Long-Term Care Facilities Management and Administration Team
What are some of the things that should be considered when you are a newly appointed manager of an assisted living or an adult day care? Some of the things that should be considered are the multidisciplinary departments, the targeted population, major staffing, human resources issues, the long-term trends that would impact the overall operation, the various forms of cooperation and integration that would exist and quality of care concerns.
I have been given the opportunity to manage the adult daycare facility PACE of Baton Rouge, and the assisted living at St. James Place in Baton Rouge, La, there are many things to consider, and I would like to begin with multidisciplinary departments. For the assisted living and the adult daycare facility, the level of care would determine the staffing needs, however, some of the available titles of staff you would need are listed below:
· Administrator- the director of the facility
· Medical Director- in charge of the overall healthcare strategy
· Director of Nursing- responsible for overseeing all nursing activities
· Admissions Coordinator- responsible for resident admissions
· Housekeeping Coordinator- responsible for all housekeeping activities
· Dining Coordinator- responsible for all dining activities
· Nursing Staff- May include registered nurses, licensed practical nurses, clinical nursing assistants and personal care technicians.
· Housekeeping, custodial, dining and administrative staff
· Activities Professional
· Social Workers
· Dietary Consultants
· Volunteers
When you look at the population that would be targeted to use these types of services, the socioeconomic trend of the older population should be considered. “Kinsella pointed out several socioeconomic indicators:
Marital status: While the current 65 and older population has a relatively low percentage of people who are divorced or separated (around 9 percent), those figures are much higher for the 45-54 and 55-64 age groups (around 18-19 percent). This means that, in the future, a much higher proportion of the older population will be divorced or separated, which will implications for caregiving and social support. Living alone a high percentage of older adults, particularly women (nearly 40 percent), live alone, which may have an implication for social support, and food delivery, etc. “
The staffing and human resources issues that could face an adult daycare and an assisted living facility are staying customer service forced. “The focus of staffing is on making the residents comfortable, happy, and safe.” (Pratt/2018., p.164). Because the adult daycare and assisted living facilities are community-based, and personnel intensive, payroll for staff is the largest portion of the budget. (Pratt/2018). There is a large need for both professional and non-professional staff working together as the care team which can be determined by the nature of the center social or health model. Adult daycare facility had more healthcare professional available, which can include physicians; nurses; physical, occupational, speech, and recreation therapists; nutritionist; and social workers. (Pratt/2018).
This can be addressed by hiring more professional assistance and focusing on the customer service provided to the resident. I would key in on developing key training programs. “Training in this area is particularly important to assisted living facilities, because employees coming from outside of healthcare or from other facilities may not be familiar with the services provided in ALFs.” (Pratt/2018., p. 165). “Increased availability and use of technology in the delivery of community-based services is possibly the most exciting trend in recent years and will continue to be soon. Advancement in technology in technology have both increased the range of services that can be provided and improved the productivity of staff.” (Pratt/2018., p. 219)
The management challenges in an assisted living facility is balancing cost, quality, and integration with other long-term care facilities. (Pratt/2018). The cost increase can be addressed by looking into different financial supplemental program for seniors, and additional Medicaid assistance. Other challenges are developing an organizational identity. A focus can be placed on tailoring the services to a resident population but making sure it’s open to both staff and residents, along with knowing what the facility should offer. (Pratt/2018). Interacting with resident is necessarily a challenge, but it usually a pleasant and rewarding experience.
Because both PACE of Baton Rouge, and the St. James Place are affiliated with an acute facilities Medicare and Medicaid would be there largest payor. However, the costs of care provided by an assisted living facility could vary; it depends on how fancy those services and the living quarters. (Pratt/2018). The cost to live in an assisted living facility is normally shared by the resident, family member, and governmental programs. Because PACE of Baton Rouge, and St. James Place both operate under the Program of All-Inclusive Care for the Elderly (PACE), which provides comprehensive medical and social services to certain frail, older people who still live in the community, and those that requires skilled nursing care. They can provide all the quality of care that is needed.
Moving from cooperation to integration is essential when it comes to the quality of care for a resident of either an assisted living or adult daycare. Cooperating with the other different entities is essential. For instance, if there is a dementia patient that in the beginning stage, and doesn’t require 24- hour care, the resident goes into another stage, which would require 24- hour care, because there is a skilled nursing facility already affiliates the resident care can easily transferred to that department within the organization. This type of joint connection it helps to contribute to the quality of care of the residents.
In closing, the management and administration team of a long-term care facility has a lot responsibility. Great leadership, critical thinking, problem-solving and ethical decision -making are of the utmost importance. “Like other health administrators, long-term care facility manager wears many hats in handling the staffing, budgets, marketing, legal, billing, record-keeping, and other business aspects of their center. Long-term care facility administrators play an important part in using their administrator expertise to improve the quality of patient care services.”
