The ten principles of case presentation.

· Introduction-Patient Bill of Rights presented (I-PBR)

· Patient Informed Consent received (PIC)

· History  (His) per ICM1 –MELT1

· Physical Examination  aka Medical Examination (ME)

· Laboratory Tests (LT)

· Differential Diagnosis (DDX)

· Additional Laboratory Tests (ALT) to choose from DDX and define the final DX

· Final Diagnosis (DX) 

· Prescription of Therapeutics (RX)

· The FDA required Pharmacogenomic tests (PhGX)

The patient presented with Iron deficiency anemia

2. PIC

The patient provided the consent.

3. History

Diagnosed with iron deficiency, Patient presents with

· Extreme fatigue

· Weakness

· Pale skin

· Chest pain, fast heartbeat or shortness of breath

· Headache, dizziness or lightheadedness

· Cold hands and feet

· Inflammation or soreness of your tongue

· Brittle nails

· Unusual cravings for non-nutritive substances, such as ice, dirt or starch

· Poor appetite, especially in infants and children with iron deficiency anemia

 

4. ME

physical exam findings of patient includes

· pale skin, gums, and nail beds

· rapid/irregular heartbeat

· rapid or uneven breathing

· unusual liver and spleen size

 

 

5. LT

· Complete blood count.

· Serum iron/ Serum ferritin/ Transferrin level, or total iron-binding capacity to measure iron levels.

· Reticulocyte count.

· Peripheral smear.

6. DDX

· thalassemia,

· sideroblastic anemias,

· some types of anemia of chronic disease

·  lead poisoning.

 

7. ALT

check your hormone levels, especially your thyroid hormone.

 

8. DX

Serum Ferritin is most common to diagnose iron deficiency anemia.

9. RX

· Take iron tablets on an empty stomach. ...

· Don't take iron with antacids. ...

· Take iron tablets with vitamin C.

· dietary supplement/ blood transfusion

· finding undelying cause

· Ferrous sulphate

10. Pharmacogenomic tests required by the FDA.
