Transitional Care for the Elderly
I. Introduction
As one age, one’s abilities reduce. Senior citizens do not have the strength and energy they had in their youth; consequentially, they need additional assistance in performing daily duties. According to the Aging in Place Organization (2021), the government has provided mechanisms and resources that the elderly can use as they age. Americans can use the resources as they transition to seniors. There are varied resources provided by local, state, and national governments that help with the elderly transition and settling.
II. Local Resources – Depending on locality, various local resources help in the transition process.
1.Area Agencies on Aging- Helps unite the elderly and aid their access to the assistance they may require, such as medication (Aging in Place
Organization, 2021).
2. Community Area support- Provides an organized support forum for the elderly.

III. State Resources – State-owned and administer resources and facilities for the elderly. A. Eldercare locator
1. Eldercare locator helps elderly, and their families identify elderly homes and facilities owned and managed by the state.

A. Physical exercise program for the elderly
1.The elderly can be involved in age-appropriate physical activity that aids in maintaining their bodies
2.physical activity in elderly homes acts as recreational as they get to interact with other seniors (Filinson & Maigret, 2017).
B. Skill acquisition- The elderly can be introduced to new appropriate skills for their age and body strength.
1.Beading and Knitting- The skills will keep them busy and help keep their brains active.
V. Conclusion
There are many local, state, and national resources that aid the transitioning of the elderly; the key is identifying a program that best fits each individual. Correct identification of elderly resources can lead to improving outcomes for the seniors in society. There is a need to continuously adjust the resources to meet the needs of the community.
