Planning is an essential part of development. For a community to develop effectively, strategic planning is unavoidable. Members of the community should be offered good services in healthcare to ensure that their wellbeing has been enhanced. Health care strategies and programs are critical since they improve the overall welfare of the community. The development and growth of a community are achieved when members of the community are healthy, and they can remain productive and active. Community assessment, therefore, involves a process of understanding and examining the strengths and concerns of a community (Lundy, Janes, Lundy, 2016). After the identification of the strengths and concerns of the community, plans for community development are made. The community should capitalize on the strengths and initiate various strategies to deal with the various community concerns.
Resources to Be Used in Community Assessment
For a community assessment to be effective and successful, various resources from the community are required. One of the available resources which could be used is the healthcare facilities and clinics. The community assessment will need to check on the health status of the community members. The records in the clinics will, therefore, help to indicate information on the health status of the community members. Another resource which is available for community assessment is community funding. The community government can fund the assessment process. This will ensure the assessment process is successful. The community members are also a good source of information and a resource to be used in the assessment process.
Assessment of Community Strengths and Concerns
One of the strengths of the community includes strong interagency collaboration. This has led to functional transactions among the agencies in the community. The community has a healthy water system which is fluoridated. This means that problems which are brought by water impurities are not available in our community. There is also easy accessibility of family planning services and education. This has ensured successful family planning activities. There is also an effective feeding program for the older adults. The older adults have been taken good care in the community.
There are also various concerns in the community. One of the major concerns is the lack of interest by the community leaders to improve health care services. The leaders are not willing to achieve healthcare promotion. There are also dysfunctional communication systems and methods which affect the programs of the community and implementation of projects. The healthcare of the community is also a big concern. The child clinics are inaccessible and unavailable which affects the welfare of the community.
Barriers to Implementation of Community Health Plans and Recommendations to Address Those Barriers
There are various challenges which the community experiences when it comes to the implementation of the community health plans. One of the major plans is lack of willingness by the leaders to implement these plans. Leaders are essential when it comes to implementation of programs and projects in the community. If they are not interested or committed, the implementation process is broken. Another barrier to implementation of these plans is lack of enough resources. The implementation process requires resources to ensure that the community health plans are successful. There is also lack of skilled workforce to implement these community health plans effectively. Lack of effective communication system has also affected the implementation of these plans (Hitchcock, Schubert, Thomas, 2013). Lack of communication leads to ineffective activities which in turn affects the implementation process.
There are various ways to which these barriers can be mitigated. One of the ways is to ensure that the community members elect leaders who have their best interests at heart. The community should elect leaders who are willing to improve their health status. The other recommendations are to increase collaboration with non-governmental organizations which would provide financial support and human capital to the implementation of these community health care plans. This will ensure that the community has a source of resources which will lead to effective implementation of the community health plans.
In conclusion, the community should be able to capitalize on their strengths in a move to ensure that community development and growth have been achieved. The community should also ensure that they have pushed for action and policies which will be used to address the community concerns (Porche, 2014). If these concerns are addressed, the wellbeing of the community members will be enhanced. The healthcare of the community is essential, and it should be improved at all costs. Improving the health care of the community will also lead to improvement of the economic status of the community and the community members.
