The health of the community members is essential to the development and growth of a community. Health is an essential component for the wellbeing of a community. The leaders and community members are expected to ensure that the various concerns related to health have been addressed. The necessary policies are supposed to be developed to ensure that the various health issues have been addressed. There is no community which is not faced with health issues. Research is therefore essential to ensure that the various issues have been determined so that the necessary measures can be developed. The various issues in a community which faces the health may include pollution, lack of clean water, bad drainage systems, lack of accessible and well-equipped child clinics, inadequate workers of health care facilities among other issues (Nardi, Petr, 2013). There are also other issues which are not necessarily related to health, and they are supposed to be tackled in the right manner. This paper will address the various the various resources which are available in the society to be used for community assessment, the various strengths and weaknesses of the community and also the barriers to implementation of community health plans.
Resources for Community Assessment
There are many resources which are available in my community which will assist in the assessment process of the community. One of the resources is the people. The community members are the best resources which will help in the assessment of the community. The community members have information which is related to the issues which affect them. They are the most critical primary resources. This is because they understand the issues and problems which face the community members. Through the use of the community members, one will be able to complete a community assessment process.
Another resource which can be used in the community assessment is the data which is contained in the community healthcare facilities. The healthcare facilities are a good source of information. They will be able to provide information on the healthcare problems which the majority of the community members are diagnosed with. The leaders of the community are also a good resource. This is because leaders are aware of the problems which face their members hence they are a good source of information. Another resource which can be used are the government officers who have information related to the activities in the community (Porche, 2014). The government offices have information regarding the various projects which have been undertaken in the community. If all these resources are well utilized, one will be able to complete the community assessment process effectively.
Community Strengths and Concerns
There are various strengths and concerns which exist in our community. One of the strengths of the community is the increased involvement of leaders in health promotion. The leaders have remained committed to ensuring that the health of the community members has been developed. The second strength includes well equipped and accessible child clinics. This is in the move to reduce child mortality and improve health care in the society (Hardcastle, Powers, Wenocur, 2011). Another strength includes effective communication between the leaders and the community members. The community members are free to express their complaints to their leaders, and the leaders are very accessible to the community members. These strengths are essential for the community.
There are also concerns in the community. One of the main concerns is the inaccessibility of the family planning services. There is also inadequate information regarding the options for family planning. Another concern is the neglect of the older adults. The feeding programs which are supposed to take care of the old adults are inaccessible and ineffective. These concerns affect the growth and the well-being of the community.
Potential Barriers to Implementation of Community Health Plans
There are barriers which exist to prevent effective implementation of community health plans. One of the barriers is discord among the leaders. If the leaders are unable to agree on the best methods to implement a plan, it becomes hard. Another barrier is corruption. This has led to lack of enough funds to fund the implementation of these plans. Lack of interagency cooperation is also a problem with the implementation of these plans (Lundy, Janes, Lundy, 2016). All these barriers have crippled the implementation of these plans.
In conclusion, these barriers can be addressed. The leaders should come together and carry out team building activities. This will enhance their relationships hence they will work together to improve community healthcare. The leaders should also set measures which will curb corruption to ensure that community resources are available. The leaders should also ensure that effective mechanism have been laid down to improve interagency relationships to enhance implementation of community health plans.
