Care coordination

Care coordination mainly involves the deliberate organizing of the patient care activities as well as sharing of information amongst all the participants who mainly are concerned with the care that is given to patients so that they can achieve more effective care (Gray, et al., 2020). This does mean that most of the patients do need preferences that are known ahead of time and that are communicated in a tight time to the right people and hence this information will be used in providing appropriate as well as care to the patients.

Care coordination mainly involves having deliberate organizing of patient care activities as well as sharing of information amongst the participants who are concerned with patients' care so that they can achieve more and effective care (Gray, et al., 2020). The main goal of care coordination is meeting the needs of the patients and the preferences which should be delivered in high-quality value healthcare. this means that the patients' needs, as well as preferences, have to be communicated at the right time as well as to the right people and hence this information will be used to guide the delivery of safe and also appropriate effective care.

Care coordination is said to be very important since it is a key strategy that has a great potential of improving the effectiveness safety as well as the efficiency of the health care system. To put care coordination into better practice and action it is important to apply changes in the general approach and also the daily routines of the medical practices that can be overwhelming even whenever it is obvious that the changes being made are going to improve the care for the patients to provide efficiency.
