Introduction

           The healthcare promotion plans are where the healthcare organizers or the nurses have to identify the specific problem in an individual or a particular population. The problem should be necessary for a community and significant to resolve. The role of nurses is substantial because they have to devise measurable goals with the outcomes. It is observed that the healthcare plans and goals are successful when the patients are appropriately guided, and follow-up is taken. The cooperation of nurses is essential, and it is good to have the complete participation of the patients.

Scenario

           A project was assigned in a small town in California, and it was to assess the complications of teen pregnancy. I was assigned to give a health promotion plan and measure performance in the next three months. I visited the town and talked to a few ladies to determine the current situation. Knowing the current situation to make a health promotion plan was essential. Otherwise, things may not work correctly.

Identify the chosen population and include demographic data.

           The chosen population is pregnant teenagers from a small town in California. The lifestyle of the participants varies from one another. The majority of them were homemakers, and others were office-going. The age of the population selected was in a narrow range from 17-19 years, and all of them were pregnant. The race and ethnicity were black and Hispanic. Their education was varied as most of them graduated. The employment and income were also diverse and were not of much significance because most of them were homemakers.

Characteristics of the group for this activity

           The characteristics of the group were similar and related to the targeted population in the following ways. The people of interest were pregnant, and most of them were about to complete the second trimester or in the initial stage of the third trimester. They lacked access to care because most were housewives and belonged to the black community. The personal income was less for those going to the office, and others faced similar issues because of their partner's lower income. Moreover, they are not guided by the complications of the teen's pregnancy. The participants' health seems not to be good enough for full-term pregnancy because of less dense nutrient foods and supplements. It was the reason for choosing the population for the activity.

Why your chosen population is predisposed to this health concern or health need, and why they can benefit from a health promotion educational plan.

           The literature shows that teen pregnancy is among the most critical health concerns. It is more important when it comes to the black population. They already lack access to healthcare. Teen pregnancy is more complex than the other age group. If the women are not provided with the proper nutrition and supplements, it becomes an issue for the life of both mother and fetus. Many authors do in-depth analyses of the problem by publishing the papers. One such study was done by Kassa (2021) to elaborate on early pregnancy complications. The cross-sectional study was conducted, and 343 pregnant women were included. The results showed that teen pregnancy usually has poor outcomes because of the social and cultural factors if they are unsupportive. The demographics data explained above and the literature cited related to the topic showed that the factors that lead to the problem include the lack of family planning ideas, methods of conception, methods of protective intercourse, financial instability, and racial discrimination (Sanford, 2018). These factors contribute to the health disparities and lack of access to services. The chosen population can benefit from the health education plan by getting awareness of family planning, proper nutrition meals, supplements, family support, and methods of conception for the next pregnancy. It is also evident in the literature and cited by (Ribas, 2021). 

Based on the health concern of your hypothetical individual or group, discuss what you would include in developing a sociogram.

           The social behaviors need to be improved because the families are based on a low income, and the participants need to adjust their nutritional and medical needs within the budget. I will include healthy supplement aids in the plan for pregnant teen women who need guidance related to these issues. The cultural considerations will be kept in mind to plan their dietary needs. I will include the foods they can quickly get from the market and meet their nutritional needs. The economic needs will be considered, and I will educate them accordingly to manage their finance and health. Most participants have sedentary lifestyle behavior, so I will include the lifestyle modifications accordingly (Loan, 2018).

Identify their potential learning needs. Collaborate with the individual or group on SMART goals that will be used to evaluate the educational session

The SMART goals for the educational session are as follows.

· The goal is to create awareness of family planning, safe use of protection, and conception methods among women in the next three months. 

· To create awareness among pregnant women to evaluate their nutritional needs during the pregnancy and meet them within the budget in the next two months. 

· To create awareness among the male partners of women to apply for Medicaid health insurance to meet the expenses of the check-up and visits to hospitals during pregnancy in the next month. 

· To induce the habit of clean eating, physical activity, and positive lifestyle changes with family support among pregnant teen women in the next three months. 

Identify the individual or group's current behaviors, outline clear expectations for this educational session, and offer suggestions for how the individual or group's needs can be met.

           The current behaviors are not encouraging. The lifestyle is unhealthy and sedentary. Most pregnant women are indulged in eating cheap less-nutrient-dense foods because their health is affected. The current behaviors are harmful because of a lack of awareness of the methods of conception and protected intercourse. The lack of understanding of family planning is also one of the crucial factors in this aspect.

Outline

Clear expectations 

· It is expected that the family support will be increased

· It is expected that the participants will have awareness related to conception and family planning 

· It is expected that the family will know the method to access the healthcare 

· It is expected to see positive changes in the lifestyle of pregnant ladies. 

Suggestions 

· It is suggested to convince your partner to have protected sex. 

· Once you get pregnant, apply for Medicaid healthcare insurance 

· Take care of the fruits and vegetables with the water intake 

· Shed the sedentary lifestyle and indulge in physical activity to have a healthy pregnancy (Ritchie, 2018). 

Health promotion goals must be clear, measurable, and appropriate for this activity. Consider objectives that will foster behavior changes and lead to the desired outcomes.

The health promotion goals are as follows.

· The health awareness sessions will create awareness among the participants to have a safe conception in the subsequent pregnancy, and the women will learn it in the next three months. 

· The educational training will guide the women to evaluate dietary needs according to their condition in the next three months. 

· The expected outcomes are a positive response from the family, and the women will get physical and emotional support in the next three months. 

Conclusion

           It is concluded that the health promotion plans help the patients and targeted population groups. Teen pregnancy is not a health disorder but a complication that is triggered because of a lack of access to health care. The health promotion will educate the women and improve the targeted population's overall health.
