Mental has been a global concern, and it has not attained attention till now. There are various mental health cases in the USA, and people have autism, dementia, bipolar disease, and depression. These conditions affect daily life and their connection with other people in the community. Therefore, the paper focuses on developing a preliminary care coordination plan by discussing mental health as a health concern. Specific goals are discussed to address a mental health problem. An effective and safe continuum of Care is also identified in this report.

There are various causes of mental health issues such as drug usage, feelings of isolation, experiences concerned with chronic medical conditions (diabetes/cancer), and early negative experiences of life such as sexual assault and child abuse.
