Nursing Care Plan

The patient care plans are essential in the healthcare organizations or nursing homes because of the several physical, psychological and cultural needs. The outdoor patients face fewer challenges because they do not have severe disorders and ailments. The nurses have to give them recommendations related to home care. Healthcare is an essential factor for patients with severe ailments. The following report will discuss the factors, including a patient's cultural, physical and psychosocial needs. The trauma is a critical disorder, and the patient needs the proper healthcare plan by the nurses.

Discussion

Physical health needs of the patient

The nursing plan for trauma patients varies from case to case. The patients have to describe their history to the nurses. It is often observed that the traumatic patients are often not cooperative with the nurses and doctors. The nurses must talk to patients' families to discuss their physical needs. The first thing in the patient care plan is to take care of the patient's mood swings. The trauma affects mental health, and it is necessary to keep the patients happy. The physical health needs of patients are essential to meet to improve their health (Ferreira, 2018).

The following important factor in physical health needs includes a balanced diet. Usually, the patients suffering from different traumatic disorders have various mood swings because they do not eat much. The diet of such patients is disturbed, and they do not have proper meals. The nurses have to consult the dieticians or nutritionists regarding the matter and plan the meals for the patients. the next thing that is important for the patients is the medication. Sometimes the patients are unwilling to take the medication, so the care should be taken.

The literature has presented the best practices to deal with the trauma for the future outcomes. It depends on the type of trauma that what kind of treatment is needed for the patient. It is essential for the patients to give the treatment in the first hour of trauma. If the trauma is because of any accident or injury, it is important for the patients to give the breathing, CPR treatment or he may need to shift into the hospital. The first hour is more critical if there is sepsis and severe injury, the wounds or inflammation can be increased so the early management is necessary (Abhilash, 2020).

Psychosocial Needs of Patients

The trauma can be because of anything, and it can be due to an accident, injury, long-term illness or an unpleasant incident in the past. The nursing plan refers to the proper calm and comfortable environment for the patients. The atmosphere around the patient is essential because it helps increase the potential to face the trauma and get relieved. For this purpose, the family is contacted and asked to keep the patient happy. The mood swings should be controlled, and if a painful memory is associated with any person, the family should avoid discussing the specific person or the issue.

Other psychosocial needs are to eliminate the habit of sleep deprivation. It is a common observation by the nurses that the patients cannot get the proper sleep after the trauma. The nurses can use the anti-depressants or sedatives as the instructions of doctors. The nurses should remember that the medication should only be devised according to the prescription written by a doctor. The nurses have to play the role of psychotherapists because such patients require the therapies and detailed explanations for a speedy recovery. The nursing care plan should adhere to these factors (Fried, 2018).

Cultural needs

Trauma-based patients usually have less awareness, and their family lacks the knowledge to handle such patients. The nursing care plan will include the proper awareness of how the patient will remain calm and how the culture impacts the patient's health. If the environment surrounding the patient is uncomfortable or the family is not supportive, the patient will have a slow recovery. The family should also be consulted to build trust. It is a common cultural practice that the patients or their families ignore the trauma or mental health in the initial stage, but there are a lot of problems that they face later.

The nursing plan also reviews the cultural barriers between the patients and nurses. If the trauma-based patient comes to the nurse, it will be evaluated whether there are cultural or language differences. The nurses must overcome such challenges and ensure the patient is in the right place. The trust between the patient and nurse is also a part of the nursing care plan because the nurses want the patients to narrate the consequences of their current condition/.

Community resources

The community resources are family support and proper check-ups. The patients suffering from trauma vary from one another. If there is a long-term trauma, the nursing care plan will go for the long-term. The family should not demotivate the patient or the nurse in the treatment process. Moreover, the community resources include the proper healthcare organizational setup where the patient can be examined and the various remedies. The hospital's infrastructure should be proper and modern to guide the patient. The nutritionists, psychotherapists and nurses must cooperate in the healthcare setting to treat such patients (Wylie, 2018).

The list of community resources are:

· Hospital infrastructure to diagnose the issue

· The therapeutic tools for the counselling

· The nurses to care the patients

· The information system to record patient data

· The monitoring and evaluation of goals and all of them are related to care continuum.

The goals are to provide the immediate short term and gradual long term treatment to the patient to improve his well-being, heal the inflammation or injury or to counsel the psychological in the given period with the coordination of nurses and healthcare organizations.

Conclusion

It is concluded that the nursing care plan for trauma-based patients depends on various factors. The physical needs of patients should be considered because their mental and physical health is deteriorating. There is a need for a proper diet and balanced nutrients with the required medication among the patients, and doctors should guide it. The community resources should be adequate, and there should be no cultural or psychosocial barriers in the treatment process. The treatment plans are often helpful in the speedy recovery of patients.
