I liked that you explained the collaboration that your interdisciplinary team needed to work towards achieving your plan’s objective. You did a good job of discussing who would be involved but also need to discuss how the team will get together to collaborate.  For a higher score, support your plan with best practices. The literature may have some ideas for collaboration that you haven't thought about. For a higher score, can you provide some real-world examples either from your practice or from the literature?
