EL Assignment #1

This assignment is required for the experiential component of the course.

1. First, complete a Home Environmental Assessment List (HEAL) survey within your own living environment. Then, conduct an assessment using the HEAL survey and create an action plan in collaboration with a willing adult in a living location other than your own. Plan to schedule a time to complete this activity via Zoom, FaceTime, or another communication tool. Students are not required to go into another person's home/living environment to complete this assignment. You will upload the completed HEAL survey and action plan related to the other adult's living environment. Please include the name, contact information, and electronic signature on the bottom of the HEAL survey.   HEAL(1).pdf

Since this is a pdf, you will need to complete using "Fill & Sign" in Adobe, or you will need to print the form, complete it, and scan it into the assignment link (suggest use of home scanner or TurboScan App).  DO NOT upload as picture images as there are many times that we cannot open these images.

2. Provide a narrative summary which includes the following:

· a statement describing the type of living environment in which the HEAL was completed (e.g., single family or multiple family, stand-alone home, condominium, duplex, apartment, mobile home, shelter, urban/rural, etc.)

· a description of lessons learned from this assignment regarding environment as a determinant of health (use book or other professional resource)

· a discussion of nursing's role and responsibility in home and broader environmental assessment

Upload all of the above into the appropriate link. 

Evaluation:

· 4 points - Completeness of HEAL survey and action plan

· 10 points -  thoughtfulness and relevancy of narrative summary

· 4 points- use of text book or other professional resource

· 2 points- APA formatting of references used, grammar, etc.

Theory Assignment #1

List the 6 current priorities, identifying the one that you think is most important for your local community. Find data online indicating why this priority is most important for your local community, and include a brief explanation of your reasoning. (20 points).

2. Find your local health department online. Supply the name and the website. Find the following:

· mission/vision/goals

· clinic services available to the public (medical or otherwise that include an HCP-- LIST AT LEAST 5 SERVICES)

· hours and location for each clinic service listed

· qualifying factors for receipt of clinic services (such as insurance, income requirements, etc.)

· non-clinic services provided by the local health department (LHD)-- LIST AT LEAST 5 SERVICES

In addition to this information, answer the following:

· Is there ability for a community member to email someone or request an appointment from the LHD website?

· Did you know that your LHD provided these clinic and non-clinic services?

· Do you think that the hours are conducive to community member work schedules?

· If not, who do you think that you would talk to about expanding services/service hours? (20 points)

Theory Assignment #2

Students should complete this assignment in a Word document. You may submit more than one document if needed to ensure that screenshots are included in the submission. This assignment does not need a title page or other APA formatting except in-text citations and references.

1. Students will need to register ahead of the due date of the assignment, however there is no cost to register and complete the module.  Students will be required to complete the program in its entirety.  There will be two files to be submitted for the assignment; one screenshot and a pdf copy of the certification of completion (see below).  The screenshot will need to be taken at the time of completion, as you cannot go back later to retrieve this information. 

a.   Screenshot one : results of the Learner Assessment II, (post-assessment quiz).  This screenshot should show the number correct of 20 points.  This screen will be shown right after the post-assessment  quiz  is submitted. Students will earn the quiz score percentage (out of 10 points instead of 20 points)

b. Copy of  pdf certification  with the student’s name.

*****Points for this question will be assigned based on the post-assessment quiz score (10 points), along with copy of the certificate of completion (10 points).

 

2. Describe the  public health responsibilities  of clinicians related to patients with diarrhea. (5 points)

3. Describe at least two responsibilities of clinicians during a foodborne outbreak. (5 points)

4. Identify two sources for accurate information during foodborne outbreaks and other public health emergencies.(5 points)

 OPTIONAL: I was given information on how to be a beta-tester for an outbreak module related to Zombies and Unicorns. It was created by an epidemiologist. It basically shows how many unicorns need to be immune in order to avoid zombification, and you can choose if these unicorns stay together or not within the herd. It is interesting, but not required to be completed.
